沙库巴曲缬沙坦联合地高辛注射液治疗急性心力衰竭的效果及对心功能的影响

摘　　要：目的观察沙库巴曲缬沙坦联合地高辛注射液治疗急性心力衰竭的效果及对心功能的影响。方法选取2021年3月—2024年3月樟树市人民医院收治的急性心力衰竭患者70例,依据随机数字表法分为研究组与对照组,每组35例。研究组给予沙库巴曲缬沙坦联合地高辛注射液治疗,对照组给予地高辛注射液治疗,2组均持续治疗1周。比较2组临床疗效,治疗前后心功能指标[心排血量(CO)、左室射血分数(LVEF)、每搏输出量(SV)]、心力衰竭标志物[心肌肌钙蛋白I(cTnI)、N末端脑钠肽前体(NT-proBNP)、可溶性生长刺激表达基因2蛋白(sST2)]、血管内皮功能指标[内皮素-1(ET-1)、一氧化氮(NO)]水平,不良反应。结果研究组总有效率(94.29%)高于对照组(74.29%)(χ^(2)=5.285,P=0.022)。治疗1周后,2组CO、LVEF、SV及NO水平较治疗前升高,cTnI、NT-proBNP、sST2及ET-1水平较治疗前降低,且研究组高/低于对照组(P<0.01)。研究组与对照组不良反应总发生率比较差异无统计学意义(5.71%vs.11.43%,χ^(2)=0.182,P=0.669)。结论沙库巴曲缬沙坦联合地高辛注射液治疗急性心力衰竭可进一步提升疗效,提高心功能,改善心力衰竭标志物水平及血管内皮功能,且用药安全性高。Objective To observe the effect of sacubactril and valsartan combined with digoxin injection on acute heart failure and its influence on cardiac function.Methods A total of 70 patients with acute heart failure admitted to Zhangshu People′s Hospital from March 2021 to March 2024 were selected,and they were divided into study group and control group according to random number table method,with 35 patients in each group.The study group was treated with sacubactril valsartan combined with digoxin injection,and the control group was treated with digoxin injection.Both groups were treated for 1 week.The clinical efficacy,cardiac function indexes(CO,LVEF,SV),heart failure markers(cTnI,NT-proBNP,sST2),vascular endothelial function indexes(ET-1,NO)levels before and after treatment,and adverse reactions of the two groups were compared.Results The total effective rate of the study group(94.29%)was higher than that of the control group(74.29%)(χ^(2)=5.285,P=0.022).After 1 week of treatment,the levels of CO,LVEF,SV and NO in two groups were higher than before treatment,the levels of cTnI,NT-proBNP,sST2 and ET-1 were lower than before treatment,and the levels in study group were higher/lower than those in control group(P<0.01).There was no significant difference in the incidence of adverse reactions between the study group and the control group(5.71%vs.11.43%,χ^(2)=0.182,P=0.669).Conclusion Sacubactril valsartan combined with digoxin injection in the treatment of acute heart failure can further enhance the efficacy,improve the heart function,improve the level of heart failure markers and vascular endothelial function,and the drug is safe.
