饮食护理干预对肝硬化并发上消化道出血后肝性脑病的影响分析

摘　　要：目的 探讨饮食护理干预对肝硬化并发上消化道出血(UGB)后肝性脑病(HE)的影响。方法 选取2022年10月—2023年6月本院接收的60例肝硬化并发UGB患者为研究对象,并随机分为对照组和观察组,每组30例。其中,对照组采用常规护理,观察组采用饮食护理干预,对比两组出血情况、临床疗效及HE发生率、生活质量。结果 观察组的再出血次数、出血量均少于对照组(P<0.05)。观察组的总有效率、HE发生率低于对照组(P<0.05)。护理后,观察组的汉密尔顿抑郁量表(HAMD)、汉密尔顿焦虑量表(HAMA)评分均低于对照组(P<0.05)。观察组的生活质量、护理满意度均高于对照组(P<0.05)。结论 针对肝硬化合并UGB患者应用饮食护理干预可减少再出血次数、出血量及HE发生率,缓解其负性情绪,提升临床疗效及生活质量,提高护理满意度,应用价值显著。Objective To explore clinical effect and prevention of hepatic encephalopathy(HE)of dietary nursing intervention for liver cirrhosis upper patients complicated with gastrointestinal bleeding(UGB).Methods The paper chose 60 liver cirrhosis patients complicated with UGB in our hospital from October 2022 to June 2023,and divided them into two groups randomly,with 30 cases in each group.Control group was treated with routine nursing,while observation group with dietary care intervention.Bleeding situation,clinical effect,incidence of HE,and life quality were compared between two groups.Results Number of rebleeding rate(1.04±0.16)and bleeding volume(412.42±20.11)mL in observation group was lower than control group(P<0.05).Total effective rate of observation group was 96.67%,and incidence of HE was 3.33%,lower than control group(P<0.05).After nursing,Hamilton Depression and Depression Scale(HAMD,HAMA)scores of observation group were lower than control group(P<0.05).Life quality and nursing satisfaction degree of observation group were higher than control group(P<0.05).Conclusion Dietary nursing intervention can reduce frequency,volume,and incidence of recurrent bleeding of patients with liver cirrhosis complicated with UGB,alleviate negative emotions,improve clinical effect,life quality and nursing satisfaction degree,which has significant application value.
