手功能康复训练仪联合手指操对冠状动脉介入术后桡动脉并发症的应用研究

摘　　要：目的 探讨手功能康复训练仪联合手指操在冠状动脉介入术后桡动脉并发症的应用价值。方法 本试验观察对象为2023年1—10月在心内科接受经桡动脉冠状动脉介入(TRI)治疗的160例患者,按照入院时间分组,将2023年1—5月的80例患者纳入对照组,并予以TRI常规术后护理,将2023年6—10月的80例患者纳入实验组,在对照组护理基础上,于术后2h加入手功能康复训练仪,同时结合手指操干预措施。比较两组患者干预前、干预后、术后12h手掌周径、指脉氧饱和度、疼痛评分、舒适度评分,以及局部血肿、局部出血、上肢肿胀和上肢麻木并发症情况。结果 实验组干预后2h、术后12h手掌周径显著低于对照组(P<0.05),实验组干预后、术后12h指脉血氧饱和度高于对照组(P<0.05)。实验组干预后、术后12h的Kolcaba舒适状况量表(GCQ)评分均高于对照组,疼痛评分低于对照组(P<0.05)。实验组并发症发生率低于对照组(P<0.05)。结论 手功能康复训练仪联合手指操能减轻TRI患者肿胀和疼痛程度,提高舒适度,减少并发症,值得临床上推广应用。Objective To explore application value of hand function rehabilitation training device combined with finger exercises in treatment of radial artery complications after coronary intervention surgery.Methods The paper chose 160 patients with radial coronary intervention(TRI)treatment in cardiology department from January to October 2023 as observation objects,and divided them into groups according to admission time.80 cases from January to May were in control group were treated with routine postoperative nursing for TRI,and 80 cases from June to October in experimental group with hand function rehabilitation training device combined with finger exercise intervention 2 hours after surgery on the basis of control group.Palm circumference,finger pulse oxygen saturation,pain scores,comfort scores,complications of local hematoma,local bleeding,upper limb swelling,and upper limb numbness were compared between two groups before and after intervention,12 hours after surgery.Results After intervention(2 hours after surgery)and 12 hours after surgery,palm circumference of experimental group was significantly lower than control group(P<0.05),while finger vein blood saturation of experimental group was higher than control group(P<0.05).Kolcaba Comfort Scale(GCQ)scores of experimental group after intervention and 12 hours after surgery were higher than control group,pain scores were lower than control group(P<0.05).Incidence of complications in experimental group was lower than control group(P<0.05).Conclusion Hand function rehabilitation training device combined with finger exercises can reduce swelling and pain,improve comfort,and reduce complications of TRI patients.
