摘 要:目的 分析盐城地区维生素D受体(VDR)基因多态性与儿童哮喘合并肺炎支原体(MP)感染的相关性,为盐城地区儿童哮喘患儿预防MP感染提供新的方向。方法 选取2021年6月—2023年12月在盐城市妇幼保健院儿科确诊为儿童哮喘的病例102例为研究对象,根据患儿是否合并MP感染分为儿童哮喘MP(+)组(n=43)和儿童哮喘MP(-)组(n=59),选取同期来本院健康体检的儿童52例作为对照组,收集三组患儿的临床及实验室资料,测定血清维生素D水平、机体炎症因子及肺功能指标水平,提取全血基因组DNA并进行基因多态性检测,采用logistic回归分析盐城地区儿童哮喘合并MP感染的危险因素。结果 儿童哮喘MP(+)组外周血中炎症因子IL-6和PCT与对照组间差异均具有统计学意义(P<0.01);儿童哮喘MP(+)组外周血中25(OH)D水平低于儿童哮喘MP(-)组和对照组(P<0.01);三组间FEV1、FVC、PEF、FEV1/FVC间差异均具有统计学意义(P<0.01),儿童哮喘MP(+)组FEV1、FEV1/FVC低于儿童哮喘MP(-)组和对照组(P<0.01),且儿童哮喘MP(+)组FVC和PEF低于儿童哮喘MP(-)组和对照组(P<0.01);三组间FokI基因型间差异均具有统计学意义(P<0.01),儿童哮喘MP(+)组VDR FokI TT基因型明显高于儿童哮喘MP(-)组和对照组(P<0.01);多因素logistic回归分析显示VDR FokI TT基因型、FVC和25(OH)D水平是盐城地区儿童哮喘合并MP感染的影响因素(P<0.01)。结论 盐城地区VDR基因中的rs2228570位点携带T等位基因会增加哮喘合并MP感染发生的风险。Objective To analyze correlation between polymorphism of vitamin D receptor(VDR)gene and mycoplasma pneumoniae(MP)infection of children with asthma in Yancheng area,and provide new directions for prevention of MP infection of children with asthma.Methods The paper chose 102 cases children with asthma in pediatrics department of Yancheng Maternal and Child Health Hospital from June 2021 to December 2023 as research subjects,and 52 children with physical examination in our hospital during the same period as control group.According to whether being infected with MP or not,children were divided into MP infection group(n=43)and non MP infection group(n=59).Clinical and laboratory data of three groups of children were collected,serum vitamin D levels,body inflammatory factors,and lung function indicators were measured.Whole blood genomic DNA extraction and gene polymorphism testing was performed,risk factors for asthma combined with MP infection of children in Yancheng area were identified with logistic regression analysis.Results There was statistically significant difference in levels of inflammatory factors IL-6 and PCT in peripheral blood of MP(+)group than control group,(P<0.01).Level of 25(OH)D in peripheral blood of children with asthma MP(+)group was lower than children with asthma MP(-)group and control group(P<0.01).Difference in FEV1,FVC,PEF,and FEV1/FVC among three groups was statistically significant,(P<0.01).The FEV1 and FEV1/FVC of MP(+)group were lower than MP(-)group and control group(P<0.01),and FVC and PEF of the MP(+)group were lower than the MP(-)group and control group(P<0.01).Difference in FokI genotypes among three groups was statistically significant.VDR FokI TT genotype in the MP(+)group was significantly higher than the MP(-)group and control group,(P<0.01).Multivariate logistic regression analysis showed,VDR FokI TT genotype,FVC,and 25(OH)D levels were influencing factors of MP infection of children with asthma in Yancheng area(P<0.01).Conclusion Carrying the T allele at rs2228570 locus in t
