青少年肱骨小头骨折的切开复位内固定治疗研究

摘　　要：目的 探讨切开复位内固定术在青少年肱骨小头骨折患者中的应用效果。方法 回顾性分析2018年1月—2023年1月本院收治的42例青少年肱骨小头骨折患者临床资料,按不同治疗方法分为对照组和观察组,每组21例。其中,对照组采取闭合复位保守治疗,观察组采取切开复位内固定术,持续随访6个月。对比两组治疗效果、肘关节功能及并发症发生率。结果 观察组治疗总有效率高于对照组;术后6个月肘关节功能评分(MEPS)中的运动功能评分、稳定性评分、日常活动评分、疼痛评分,均高于对照组;并发症发生率低于对照组;上述差异均有统计学意义(P<0.05)。结论 切开复位内固定术对于促进青少年肱骨小头骨折患者的肘关节功能恢复具有积极作用,并有助于降低并发症的发生率。Objective To explore application effect of open reduction and internal fixation for capitulum humeral fracture of adolescent patients.Methods The paper reviewed and analyzed clinical data of 42 adolescent patients with capitulum humeral fracture in our hospital from January 2018 to January 2023,and divided them into two groups according to different treatment methods,with 21 cases in each group.Control group was treated with conservative treatment of closed reduction,while observation group with open reduction and internal fixation surgery,with continuous follow-up of 6 months.Curative effect,elbow joint function and incidence of complications was compared between two groups.Results Total effective rate of observation group was 95.24%,higher than 66.67%in control group.The motor function scores(15.26±2.18),stability scores(8.45±0.40),daily activity scores(48.56±4.01),and pain scores(40.07±2.27),6-month postoperative elbow joint function scores(MEPS)in observation group were higher than(12.55±2.34),(7.77±0.65),(42.33±3.28),and(36.15±2.56)in control group.Incidence of complications was 0,lower than 28.57%in control group,difference was statistically significant(P<0.05).Conclusion Open reduction and internal fixation surgery can help rehabilitation of elbow joint function and reduce incidence of complications of adolescents with capitulum humeral head fracture.
