摘　　要：目的 探讨术前纤维蛋白原水平与心脏瓣膜术后住院时间延长的相关性。方法 连续性选取2019年1—6月广东省人民医院心外重症监护二区收治的254例心脏瓣膜术后患者,以术后住院时间延长作为结局事件,将术前纤维蛋白原水平按照四分位法分为Q1组、Q2组、Q3组、Q4组,通过单因素和多因素logistic回归模型分析纤维蛋白原水平与术后住院时间延长的相关性。结果 四组患者在住院时间延长事件的发生率上差异有统计学意义(P=0.026);四组患者住院死亡率差异无统计学意义(P>0.05)。多因素Logistic回归分析提示,以纤维蛋白原Q1组为参考,Q4组患者发生住院时间延长这个结局事件的OR值为2.95,95%置信区间1.10~7.95,差异有统计学意义(P=0.32)。结论 术前纤维蛋白原水平与心脏瓣膜术后住院时间的延长存在关联。Objective To explore correlation between preoperative fibrinogen levels and prolonged hospital stay after heart valve surgery.Methods The paper chose 254 patients with heart valve surgery in the Extracardiac Intensive Care Unit of Guangdong Provincial People's Hospital from January to June 2019 consecutively,and divided them into four groups with quartile method based on preoperative fibrinogen levels with outcome event of prolonged hospital stay after surgery.Correlation between fibrinogen and prolonged hospital stay after surgery was analyzed with univariate and multivariate logistic regression models.Results 254 patients were included in the study,and were divided into four groups with quartile method according to preoperative serum fibrinogen levels.Incidence of prolonged hospitalization events was statistically significant among the four groups(P=0.026).There was no statistically significant difference in in-hospital mortality rate among four groups(P>0.05).Multivariate logistic regression analysis showed,with reference of fibrinogen Q1 group,OR value for outcome event of prolonged hospital stay in Q4 group was 2.95,with 95%confidence interval of 1.10~7.95,which was statistically significant(P=0.32).Conclusion Preoperative fibrinogen levels are correlated with prolonged hospital stay after heart valve surgery.
