益气解毒方联合阿昔洛韦治疗复发性生殖器疱疹患者的疗效及对其血清炎性因子和机体免疫功能的影响

摘　　要：目的探讨益气解毒方联合阿昔洛韦治疗复发性生殖器疱疹(Recurrent genital herpes,RGH)患者的疗效及对其血清炎性因子和机体免疫功能的影响。方法选取2020年4月—2021年2月期间常熟市中医院收治的RGH患者86例,按照随机数字表法分为对照组和观察组,每组各43例。对照组口服阿昔洛韦治疗,观察组在阿昔洛韦治疗基础上联合益气解毒方治疗,以4周为1个疗程,共治疗3个疗程。观察比较两组患者症状缓解时间、临床疗效、疾病复发状况,治疗前后血清炎性细胞因子[白介素-2(Interleukin-2,IL-2)、白介素-13(Interleukin-13,IL-13)、γ-干扰素(Interferon gamma,INF-γ)]水平、免疫功能指标(CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+))及不良反应发生情况。结果治疗后观察组患者水疱吸收起效时间、水疱发作持续时间、全身症状消失时间、水疱结痂时间均短于对照组,差异有统计学意义(P<0.05)。治疗后两组患者血清IL-2、INF-γ水平较治疗前升高,IL-13水平较治疗前降低,差异有统计学意义(P<0.05);且观察组血清IL-2、INF-γ水平明显高于对照组,IL-13水平明显低于对照组,差异有统计学意义(P<0.05)。治疗后两组患者CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)较治疗前升高,CD8^(+)较治疗前降低,差异有统计学意义(P<0.05);且观察组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)均明显高于对照组,CD8^(+)明显低于对照组,差异有统计学意义(P<0.05)。治疗后观察组临床总有效率93.02%(40/43)明显高于对照组74.42%(32/43),差异有统计学意义(P<0.05)。治疗后6、12个月观察组复发率2.33%(1/43)、6.98%(3/43)明显低于对照组13.95%(6/43)、25.58%(11/43),差异有统计学意义(P<0.05)。治疗期间,两组患者不良反应发生率比较,差异无统计学意义(P>0.05)。结论益气解毒方联合阿昔洛韦能够纠正复发性生殖器疱疹患者机体免疫功能紊乱,调节血清炎性细胞因子,能够有助于改善患者临床症状,Objective To explore the effects of Yiqi Jiedu Formula combined with acyclovir on the serum levels of inflammatory cytokines and immune functions in the patients with recurrent genital herpes(RGH).Methods Eighty-six patients with RGH treated in the Changshu Hospital of Traditional Chinese Medicine from April 2020 to February 2021 were selected and assigned with the random number table method into control and observation groups(n=43).The control group was treated with acyclovir orally,and the observation group was treated with a combination of acyclovir and Yiqi Jiedu Formula.Each course of treatment lasted for 4 weeks,and both groups were treated for 3 courses.The time to symptom relief,clinical efficacy,and disease recurrence were observed and compared between the two groups.The serum levels of inflammatory cytokines[interleukin-2(IL-2),interleukin-13(IL-13),and interferon gamma(INF-γ)]and immune function indicators(CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+))were measured before and after treatment.The occurrence of adverse reactions during treatment was recorded.Results After treatment,the time to blister absorption,duration of blister onset,time to disappearance of systemic symptoms,and time for blister scab formation became shortened in both groups and were shorter in the observation group than in the control group(P<0.05).The treatment elevated the serum levels of IL-2 and IFN-γand lowered the level of IL-13(P<0.05),and the observation group had higher serum levels of IL-2 and IFN-γand lower level of IL-13 than the control group after treatment(P<0.05).The levels of CD3^(+),CD4^(+),and CD4^(+)/CD8^(+)in both groups of patients increased,while that of CD8^(+)decreased after treatment(P<0.05).Moreover,the observation group had higher levels of CD3^(+),CD4^(+),and CD4^(+)/CD8^(+)and lower level of CD8^(+)than the control group(P<0.05).The total response rate in the observation group was 93.02%(40/43),which was higher than that(74.42%,32/43)in the control group(P<0.05).The recurrence rates 6 and 12 months after
