摘 要:目的探讨自体脂肪移植联合二氧化碳点阵激光治疗烧伤后增生性瘢痕患者的影响。方法回顾性分析2021年5月至2023年10月郑州阳城医院收治的126例烧伤后增生性瘢痕患者的临床资料,按照治疗方法不同分为点阵激光组和联合组,各63例。点阵激光组患者给予二氧化碳点阵激光治疗,联合组患者给予二氧化碳点阵激光联合自体脂肪移植治疗。比较两组患者改良温哥华瘢痕评定量表评分、瘢痕厚度、血流变化、临床疗效及不良反应。结果治疗后,两组患者改良温哥华瘢痕评定量表中各维度评分及瘢痕厚度、血流灌注量均较治疗前下降,且联合组低于点阵激光组,差异有统计学意义(P<0.05);联合组患者临床总有效率(92.06%)高于点阵激光组(77.78%),差异有统计学意义(P<0.05);两组患者不良反应总发生率比较差异无统计学意义(P>0.05)。结论自体脂肪移植联合二氧化碳点阵激光治疗烧伤后增生性瘢痕,能够改善瘢痕外观、厚度及充血情况,提高临床疗效,且安全性高。Objective To investigate the effect of autologous fat transplantation combined with carbon dioxide dot matrix laser treatment on patients with hypertrophic scars after burns.Methods A retrospective analysis was conducted on the clinical data of 126 patients with hypertrophic scars after burns admitted to Zhengzhou Yangcheng Hospital from May 2021 to October 2023.According to different treatment methods,they were divided into a dot matrix laser group and a combination group,with 63 cases in each group.The patients in the dot matrix laser group were treated with carbon dioxide dot matrix laser,while the patients in the combination group were treated with carbon dioxide dot matrix laser combined with autologous fat transplantation.Compare the modifified Vancouver Scar Assessment Scale scores,scar thickness,blood flflow changes,clinical effificacy,and adverse reactions between two groups of patients.Results After treatment,the scores of each dimension,scar thickness,and blood flflow perfusion in the modifified Vancouver Scar Assessment Scale for both groups of patients decreased compared to before treatment,and the combined group was lower than the dot matrix laser group,with statistical signifificance(P<0.05);The total clinical effective rate of the combined group patients(92.06%)was higher than that of the dot matrix laser group(77.78%),and the difference was statistically signifificant(P<0.05);There was no statistically signifificant difference in the total incidence of adverse reactions between the two groups of patients(P>0.05).Conclusions The combination of autologous fat transplantation and carbon dioxide dot matrix laser therapy can improve the appearance,thickness,and congestion of hypertrophic scars after burns,enhance clinical effificacy,and have high safety.
