“辨构论治”与“以痛为腧”诊疗经筋病刍议

摘　　要：“以痛为腧”理念多以局部、近部取穴治疗软伤类痛症,效若桴鼓,自阐发以来一直被历代医家用为治疗经筋病的首方。然而在时下应用中,却逐渐凸显出一定局限,整体治疗效果差强人意。对古今经筋病之病因、病位及诊断要点等方面阐幽探赜,剖论“辨构论治”实以“以痛为腧”为纲,灵活运用、辨证传承。分析病处解剖结构、病理变化,寻伤疾之“标”“本”“因”,以期为临床治疗提供理论支持。Taking pain locations as acupuncture points,a prominent and effective treatment method for pains caused by meridian sinew disorders,has been valued by traditional Chinese medicine practitioners since it was first initiated.However,in its current applications,this treatment method has gradually shown some limitations,having an unsatisfactory efficacy as a whole.This article summarizes the etiology,locations,diagnosis and treatment of meridian sinew disorders in ancient and modern times,clarifying the relationship between the two concepts of"administrating a treatment based on structure differentiation"and"taking pain locations as acupuncture points".By analyzing the anatomical structure and pathological changes of an affected site in meridian sinew disorders,it is expected to explore its manifestations,root causes and overt causes,in order to provide theoretical support for its clinical treatment.
