朱世增基于“治风先治血”理论分期辨治支气管哮喘经验

摘　　要：朱世增认为支气管哮喘的病机关键是风邪和瘀血,提出风是内外邪风,血是络脉瘀血和脏腑血虚,基于“治风先治血”理论提出“诸般哮证皆因气壅,止哮贵在理血息风”,主张分期论治。哮即发主攻邪,发作期息风活血,化瘀定喘,常以血府逐瘀汤、桃红四物汤为基础,配合“风药”与“血药”进行加减治疗;哮已缓主固本,缓解期养血和血,固护营卫,常用玉屏风散、肾气丸,配合蛤蚧与皂荚、熟地黄与何首乌等对药辨证施治,临床疗效显著。Professor Zhu Shizeng believes that the key to the pathogenesis of bronchial asthma is wind and blood stasis,with wind here referring to internal and external pathogenic wind,while blood to blood stasis in collaterals and blood deficiency in zang-fu viscera.Based on the theory of"treating blood disorders prior to expelling wind",he proposes that"all the asthma syndromes are caused by qi congestion,and the key to stopping asthma lies in regulating blood and calming endogenous wind",advocating a staged approach to its treatment.As asthma breaks out,the treatment priority lies in eliminating the wind pathogen,calming endogenous wind and activating blood,resolving blood stasis to relieve dyspnea,usually treating the disease with Xuefu Zhuyu Tang and Taohong Siwu Tang,combined with modified"wind medicines"and"blood medicines".As asthma has been relieved,a priority is placed on consolidating the body resistance,nourishing blood,consolidating nutrient qi and defensive qi,treating the disease commonly with Yupingfeng San and Shenqi Pill,combined with couplet medicines of tokay gecko and Chinese honey locust,prepared rehmannia root and fleeceflower root.The treatment is carried out by stages and based on syndrome differentiation,usually leading to a remarkable clinical effect.
