葛根汤合天麻钩藤饮治疗脑卒中后痉挛性偏瘫临床研究

摘　　要：目的观察葛根汤合天麻钩藤饮治疗脑卒中后痉挛性偏瘫(SPAS)患者临床疗效。方法将我院120例SPAS患者随机分为对照组与实验组各60例。对照组接受常规神经内科治疗联合康复训练,实验组在对照组的治疗基础上联合葛根汤合天麻钩藤饮治疗。对比两组患者临床疗效、生活质量、凝血指标及肌电图动态变化。结果治疗后,实验组临床总有效率明显高于对照组(P<0.05);两组健康状况调查问卷-36(SF-36)各维度评分明显高于治疗前,实验组SF-36各维度评分明显高于对照组(P<0.05);两组凝血酶原时间(PT)、凝血酶时间(TT)及活化部分凝血活酶时间(APTT)明显高于治疗前,纤维蛋白原(FBG)水平明显低于治疗前,实验组PT、TT及APTT明显高于对照组,FBG水平明显低于对照组(P<0.05);两组正中神经、桡神经和尺神经的运动神经传导速度(MCV)、运动电位波幅(MPA)明显高于治疗前,MPL明显低于治疗前,实验组正中神经、桡神经和尺神经的MCV、MPA明显高于对照组,正中神经、桡神经的运动电位潜伏期(MPL)均明显低于对照组(P<0.05)。结论葛根汤合天麻钩藤饮治疗SPAS疗效显著,可提高患者生活质量,改善患者凝血指标,促进神经功能恢复。Objective To observe the clinical effect of Gegen Decoction combined with Tianma Gouteng Decoction on spastic hemiplegia(SPAS)after stroke.Methods A total of 120 patients with SPAS were randomly divided into control group and experimental group with 60 cases in each group.Control group received routine neurological treatment combined with rehabilitation training,and experimental group was treated with Gegen Decoction and Tianma Gouteng Decoction on the basis of control group.The clinical efficacy,quality of life,coagulation indexes and dynamic changes of electromyography were compared between the two groups.Results After treatment,the clinical efficacy of experimental group was significantly higher than that of control group(P<0.05);the scores of each dimension of SF-36 in both groups were significantly higher than those before treatment,the scores of SF-36 dimensions in experimental group were significantly higher than those in control group(P<0.05);the prothrombin time(PT),thrombin time(TT),and activated partial thromboplastin time(APTT)in both groups were significantly higher than those before treatment,while the FBG level was significantly lower than that before treatment,the PT,TT,and APTT in experimental group were significantly higher than those in control group,while the fibrinogen(FBG)level was significantly lower(P<0.05);the motor nerve conduction velocity(MCV)and motor potential amplitude(MPA)of the median nerve,radial nerve,and ulnar nerve in both groups were significantly higher than those before treatment,and the motor potential latency(MPL)was significantly lower than that before treatment,and the above indexes were superior in observation group to those in control group(P<0.05).Conclusion Gegen Decoction combined with Tianma Gouteng Decoction is effective in SPAS,which is helpful to improve the quality of life,improve coagulation index and promote the recovery of neurological function.
