自拟养血通络方联合八脉交会穴针刺治疗糖尿病周围神经病变临床观察

摘　　要：目的观察自拟养血通络方联合八脉交会穴针刺治疗糖尿病周围神经病变(DPN)的临床疗效及对神经传导速度的影响。方法将我院90例DPN患者随机分为观察组与对照组各45例。对照组给予常规降糖、营养神经、甲钴胺片等常规治疗,观察组在对照组的治疗基础上给予自拟养血通络方联合八脉交会穴针刺。比较两组临床疗效、中医症状评分、多伦多临床评分系统评分(TCSS)、震动感觉阈值[双侧下肢震动感觉阈值检查评分(VPT)]、神经传导速度[正中神经与腓总神经的运动传导速度(MNCV)、感觉传导速度(SNCV)]、血液流变学指标[全血高切黏度、全血低切黏度、血浆黏度、红细胞沉降率(ESR)]及不良反应情况。结果治疗后,观察组治疗总有效率高于对照组(P<0.05);两组中医药症状评分、TCSS评分、左右侧VPT、全血高切黏度、全血低切黏度、血浆黏度、ESR均降低(P<0.05),且观察组上述指标均低于对照组(P<0.05);两组正中神经、腓总神经MNCV、SNCV均升高(P<0.05),且观察组上述指标均高于对照组(P<0.05);两组不良反应比较无显著差异(P>0.05)。结论自拟养血通络方联合八脉交会穴针刺用于DPN治疗中疗效确切,可减轻症状严重程度,提升下肢震动感觉阈值与神经传导速度,降低血液黏稠度,且安全性良好。Objective To observe the clinical efficacy of self-made Yangxue Tongluo Decoction combined with acupuncture at eight confluent acupoints in treatment of diabetic peripheral neuropathy(DPN)and its influence on nerve conduction velocity.Methods Ninety DPN patients were randomly divided into observation group and control group,with 45 cases in each group.Control group was given conventional treatments such as conventional glucose reduction,nutritional nerve and mecobalamin tablets,and observation group was given self-made Yangxue Tongluo Decoction on the basis of control group.The clinical efficacy,TCM symptom scores,Toronto Clinical Scoring System score(TCSS),vibration perception threshold[bilateral lower limb vibration perception threshold test score(VPT)],nerve conduction velocity[motor nerve conduction velocity(MNCV)and sensory nerve conduction velocity(SNCV)of median nerve and common peroneal nerve]and hemorheological indicators[whole blood high-shear viscosity,whole blood low-shear viscosity,plasma viscosity,erythrocyte sedimentation rate(ESR)],and adverse reactions were compared between the two groups.Results The total effective rate of treatment in observation group was higher than that in control group(P<0.05).After treatment,the TCM scores,TCSS score,left and right VPT,whole blood high-shear viscosity,whole blood low-shear viscosity,plasma viscosity and ESR were decreased in both groups(P<0.05),and were lower in observation group than control group(P<0.05);the MNCV and SNCV of median nerve and common peroneal nerve in both groups were risen(P<0.05),and were higher in observation group than control group(P<0.05);there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Self-made Yangxue Tongluo Decoction combined with acupuncture at eight confluent acupoints has an exact efficacy in the treatment of DPN.It can relieve the severity of symptoms,enhance the lower limb vibration perception threshold and nerve conduction velocity,and reduce the blood visc
