消化道恶性肿瘤伴抑郁焦虑患者274例的中医面、舌、脉象特征及客观化分析

摘　　要：目的:客观化分析消化道恶性肿瘤伴抑郁焦虑患者的面色、舌象、脉象特征,为中医辨证提供依据。方法:应用中医四诊仪采集274例恶性肿瘤伴抑郁焦虑患者(观察组)和50名正常人群(对照组)进行中医面色、舌象、脉象特征对比分析。结果:观察组面色图中,额头的HSV_S、RGB_R、RGB_B、RGB_G显著低于对照组(P<0.01,P<0.05);观察组左脸颊、唇色的RGB_R、RGB_B、RGB_G显著低于对照组(P<0.05);左眼眶的观察组RGB_R、RGB_B、HSV_H、HSV_S、HSV_V显著低于对照组(P<0.05);右眼眶的观察组RGB_R、HSV_V显著低于对照组(P<0.05);观察组面色整体图RGB_R、RGB_G、RGB_B、HSV_V显著低于对照组(P<0.05)。说明观察组比对照组面色黑、颜色深暗,提示有气血两虚兼血瘀。观察组舌象图中全舌图、舌中、舌根图参数HSV_H显著高于对照组(P<0.01),全舌图的RGB_R、RGB_G、RGB_B显著高于对照组(P<0.05);观察组舌根图的RGB_R、RGB_B显著低于对照组(P<0.01);观察组的苔色图的RGB_R显著低于对照组,HSV_H显著高于对照组(P<0.05)。说明观察组比对照组舌色黯,苔色黄,提示有血瘀、湿热。观察组脉诊H3、H1参数显著高于对照组(P<0.05,P<0.01)。说明消化道恶性肿瘤伴抑郁焦虑患者脉象偏弦,提示心气不足、气滞血瘀。结论:中医面色、舌象、脉象的客观化参数分析显示消化道恶性肿瘤伴抑郁焦虑病机为本虚(气血两虚、心气不足)、标实(湿热内阻、气滞血瘀)。Objective:to analyze the characteristics of face,tongue and pulse in patients with gastrointestinal malignant tumors with depression and anxiety,and to provide basis for traditional Chinese medicine(TCM)syndrome differentiation.Methods:A total of 274 cases of malignant tumor patients with depression and anxiety were collected by the TCM four diagnostic instruments and the characteristics of TCM face,tongue and pulse were compared and analyzed between the observation group and the control group of 50 normal people.Results:The HSV_S,RGB_R,RGB_B,RGB_G of the forehead in the observation group were significantly lower than those of the control group(P<0.01,P<0.05).The RGB_R,RGB_B and RGB_G of left cheek and lip color in the observation group were lower than those in the control group(P<0.05).The RGB_R,RGB_B,HSV_H,HSV_S and HSV_V of the left orbital of the observation group were significantly lower than those of the control group(P<0.05).The RGB_R and HSV_V of the right orbital of the observation group were significantly lower than those of the control group(P<0.05).The overall complexion RGB_R,RGB_G,RGB_B and HSV_V of the observation group were significantly lower than those of the control group(P<0.05).The face’s color of the observation group was darker than that of the control group,indicating that there were deficiency of qi and blood combined with blood stasis.The HSV_H parameters of whole tongue,middle tongue and root tongue of the observation group were significantly higher than those of the control group(P<0.01),the RGB_R,RGB_G and RGB_B of whole tongue of the observation group were significantly higher than those of the control group(P<0.05).The RGB_R and RGB_B of tongue root map in the observation group were significantly lower than those in the control group(P<0.01).The RGB_R of the observation group was significantly lower than that of the control group,and the HSV_H of the control group was significantly higher(P<0.05).It shows that the tongue color of the observation group is darker than that of the co
