蒌丹理肺颗粒治疗慢性阻塞性肺疾病急性加重期痰热壅肺证患者的疗效观察

摘　　要：目的:观察蒌丹理肺颗粒对慢性阻塞性肺疾病急性加重期(AECOPD)痰热壅肺证患者临床症状、生活质量和炎症指标的影响。方法:采用随机、双盲、安慰剂对照研究共纳入72例AECOPD患者,随机分为治疗组和对照组,每组36例。治疗组给予蒌丹理肺颗粒口服,对照组给予蒌丹理肺颗粒安慰剂口服,两组均连续用药7d。观察两组患者临床疗效、中医证候积分、肺功能、炎症指标的改善情况。结果:经7d治疗,治疗组患者CAT评分显著低于对照组(P<0.05);治疗组显控率[65.63%(21/32)]显著高于对照组[36.67%(11/30)](P<0.05);在改善EXACT-PRO量表方面,治疗组显著优于对照组(P<0.01);单项中医证候方面治疗组患者咳嗽、咳痰、喘息、胸闷、烦躁、乏力评分显著低于对照组(P<0.05);肺功能方面,经治疗后患者在FEV1及FVC方面具有一定程度的提升,但两组组间比较差异无统计学意义;经治疗后,治疗组外周血炎症指标TNF-α、IL-17A及IL-6显著低于对照组(P<0.05)。结论:蒌丹理肺颗粒可以改善AECOPD痰热壅肺证患者临床症状,降低急性加重对肺功能带来的影响及炎症反应。Objective:To observe the effects of Loudan Lifei Granules on clinical symptoms,quality of life,and inflammation indices in acute exacerbation of chronic obstructive pulmonary disease(AECOPD)patients with phlegm-heat obstructing lung syndrome.Methods:A randomized,double-blind,placebo-controlled study was conducted on 72 AECOPD patients,who were randomly divided into a treatment group and a control group,with 36 patients in each group.The treatment group was given oral administration of Loudan Lifei Granules,while the control group was given placebo of Loudan Lifei Granules.Both groups received continuous medication for 7 days.Observe the improvement of clinical efficacy,traditional Chinese medicine syndrome scores,lung function,and inflammation indices in two groups of patients.Results:After 7 days of treatment,the CAT scores of patients in the treatment group were significantly lower than those in the control group(P<0.05);the control rate of the treatment group was 65.63%(21/32),significantly higher than 36.67%(11/30)of the control group(P<0.05).In terms of improving the EXACT-PRO scale,the treatment group was significantly better than the control group,and the difference was statistically significant(P<0.01);the scores of cough,expectoration,wheezing,chest tightness,irritability,and fatigue in the treatment group were significantly lower than those in the control group in terms of single TCM syndrome(P<0.05).In terms of lung function,patients showed a certain degree of improvement in FEV1 and FVC after treatment,but there was no statistically significant difference between the two groups.After treatment the levels of peripheral blood inflammation index TNF-α,IL-17A and IL-6 in treatment group were significantly lower than the control group(P<0.05).Conclusion:Loudan Lifei Granules can improve the clinical symptoms of AECOPD patients with phlegm-heat obstructing lung syndrome,reduce the impact of acute exacerbation on lung function and inflammatory reactions.
