摘  要:目的:探讨低密度中性粒细胞(LDGs)比例及其表面主要免疫表型分子与不明原因早期自然流产的相关性。方法:选取2022-09-2024-07江西省妇幼保健院医院收治的不明原因早期自然流产患者59例,其中不明原因早期首发自然流产患者30例(设为UESA组),不明原因早期复发性自然流产患者29例(设为URSA组)。同时选取同期、同年龄段的正常早孕女性30例作为对照组(设为HC组)。运用流式细胞术对各研究对象外周血单个核细胞(PBMCs)中LDGs的比例及其表面主要免疫表型分子(包括氧化型低密度脂蛋白受体1(LOX-1)、细胞程序性死亡-配体1(PD-L1)和CD16的表达水平进行检测,此外,对外周血T淋巴细胞亚群、调节性T细胞(Treg)的水平也一并检测,采用Logistic回归分析LDGs比例及其表面主要免疫表型分子与孕妇早期不明原因自然流产的相关性。结果:与HC组比较,不明原因早期自然流产患者外周血LDGs的水平显著升高,且URSA组又显著高于UESA组(P<0.05)。URSA组患者和UESA组患者LOX-1和CD16表达水平明显高于HC组(P<0.05)。UESE组患者PD-L1水平高于HC组(P<0.05)。组间对比分析结果显示,URSA组总T淋巴细胞计数、CD8^(+)T细胞低于HC组(P均<0.05),CD4^(+)T/CD8^(+)T细胞高于HC组。URSA组Treg细胞和总T淋巴细胞计数低于UESA组和HC组,差异均有统计学意义(P<0.05)。多因素Logistic回归分析显示LDGs为不明原因早期自然流产的独立危险因素。结论:LDGs细胞可能通过影响母体免疫状态参与并维持了不明原因早期自然流产的反复发生,是不明原因早期自然流产的独立危险因素。Objective:To explore the correlation between the proportion of low-density neutrophils(LDG)and major immunophenotypic molecules and unexplained early spontaneous abortion in pregnant women.Method:59 patients with unexplained early spontaneous abortion from September 2022 to July 2024 were selected as the observation group(including 30 patients with unexplained early spontaneous abortion and 29 patients with unexplained recurrent spontaneous abortion),while 30 healthy early pregnant women in the same age group were selected as the control group.Peripheral blood from each study subject was collected and isolated peripheral blood mononuclear cells(PBMCs),and flow cytometry was used to detect the proportion of low-density neutrophils and the expression levels of major immunophenotypic molecules on their surface,including oxidized low-density lipoprotein receptor 1(LOX-1),programmed cell death ligand 1(PD-L1),and CD16.At the same time,the levels of peripheral blood T lymphocyte subsets and regulatory T cells(Treg)were detected.The correlation between the proportion of LDGs and its surface major immunophenotypic molecules and unexplained early spontaneous abortion was analyzed in pregnant women.Results:Compared with healthy early pregnant women,the levels of LDGs in the peripheral blood of patients with unexplained early spontaneous abortion were significantly increased,and the unexplained recurrent spontaneous abortion group(URSA group)was significantly higher than the unexplained early spontaneous abortion group(UESA group).The expression levels of LOX-1 and CD16 in URSA group patients and UESA group patients were significantly higher than those in HC group women(P<0.05).The total T lymphocyte count and CD8^(+)T cells of URSA group patients were lower than those of HC group women(all P<0.05).The expression level of PD-L1 in UESA group was higher than that in HC group(P<0.05).and CD4^(+)T/CD8^(+)T cells were higher than those of HC group women.The Treg cell and total T lymphocyte counts in the URSA group were lower th
