单中心799例肾结石成分以及结石形成相关因素分析

摘  要:目的:分析近十年诊治于北京地坛医院的肾结石患者结石成分,为北京地坛医院周边肾结石的临床防治提供参考。方法:收集北京地坛医院2012年1月1日—2021年5月1日肾结石患者的临床资料,对符合纳入标准的患者的结石标本通过红外光谱法进行成分测定。结果:共收集799例肾结石患者,其中男575例,女224例;未成年人、青年、中年、老年患者比例为0.02∶1.72∶1.73∶1。六水磷酸铵镁在男女患者间分布上的差异有统计学意义(P<0.05)。体重指数(body mass index,BMI)<18.5 kg/m^(2)的患者23例,BMI 18.5~23.9 kg/m^(2)患者264例,BMI 24.0~27.9 kg/m^(2)患者346例,BMI≥28 kg/m^(2)的患者166例。BMI<28.0 kg/m^(2)或≥28.0 kg/m^(2)的2组患者结石类型差异有统计学意义(P<0.05)。结论:近十年北京地坛医院收治的肾结石患者中,中青年发病率高,结石成分以含钙类结石为主,BMI影响结石成分类型。本研究通过了解本地区患者肾结石成分以及肾结石形成的相关因素,对于肾结石患者可以通过重点防控中青年人群,改善饮食结构减少草酸钙类食物摄入,达到对本地区肾结石的有效预防。Objective To analyze the composition of kidney stones in patients diagnosed and treated in Beijing Ditan Hospital in recent ten years,and to provide reference for the clinical prevention and treatment of kidney stones around Beijing Ditan Hospital.Methods The clinical data of kidney stone patients from Beijing Ditan Hospital from January 1st,2012 to May 1st,2021 were collected,and the stone samples of patients who met the inclusion criteria were determined by infrared spectroscopy.Results A total of 799 kidney stone patients were collected,among which the proportion of minors,young,middle-aged and elderly patients was 0.02∶1.72∶1.73∶1.There were 575 males and 224 females.The distribution of magnesium ammonium phosphate hexahydrate was significantly different between male and female patients(P<0.05).There were 23 patients with body mass index(BMI)<18.5 kg/m^(2),264 patients with BMI 18.5-23.9 kg/m^(2),346 patients with BMI 24.0-27.9 kg/m^(2),and 166 patients with BMI≥28.0 kg/m^(2).There existed statistical differences in stone types between the two groups(BMI<28.0 kg/m^(2) and BMI≥28.0 kg/m^(2))(P<0.05).Conclusion In the past ten years,patients with kidney stones admitted to Beijing Ditan Hospital were high in young and middle-aged.The composition of calculi is mainly calcium-containing calculi.BMI index affected the type of stone composition.By understanding the components of kidney stones and related factors in formation of kidney stones in this region,this study can focus on high-risk middle-aged and young people,improve diet structure and reduce intake of foods high in calcium oxalate,and achieve effective prevention of kidney stones in this region.
