中医治疗紫杉类药物诱导周围神经病变的研究进展

摘　　要：紫杉类药物是一种常见的肿瘤化疗药物,被广泛应用于多种恶性肿瘤治疗中。紫杉类药物虽然可提高恶性肿瘤患者的生存率,但在治疗过程中也会出现一定不良反应,紫杉类药物诱导周围神经病变(TIPN)是紫杉类药物的常见不良反应,TIPN多表现为感觉异常、麻木和疼痛,甚至可出现运动神经元受累等临床症状,可导致药物治疗中止,影响药物治疗结局,且严重影响患者的生活质量,目前日益受到临床重视。TIPN治疗指南中的推荐治疗用药仅有度洛西汀,其他临床常用药物,如谷胱甘肽、奥卡西平等药物,以及一些非药物治疗的效果缺乏充分证据。中医在TIPN的防治中表现出明显优势,但研究较少且缺乏系统总结,文章通过对中药方剂、中药注射液、针灸等手段治疗TIPN进行综述,从现代医学中药理、分子生物学层面进行分析,为中医治疗TIPN提供依据,以期增强TIPN患者的预防和治疗效果。Taxanes are a common class of chemotherapy drugs widely used in the treatment of various malignancies.While they can improve survival rates in cancer patients,taxanes also cause certain adverse effects during treatment.Taxane-induced peripheral neuropathy(TIPN)is a frequent adverse events,often presenting as sensory abnormalities,numbness,and pain,with some cases involving motor neuron impairment.These symptoms can lead to the discontinuation of treatment,negatively affecting outcomes and severely impacting the patient's quality of life,which has drawn increasing clinical attention.Currently,duloxetine is the only recommended drug for TIPN in treatment guidelines,while other commonly used drugs,such as glutathione and oxcarbazepine,and non-drug therapies,lack sufficient evidence.Traditional Chinese medicine(TCM)has shown significant advantages in preventing and treating TIPN,but research is limited and lacks comprehensive review.This article provides an overview of TCM approaches,including herbal formulas,injections,and acupuncture,analyzing them from the perspectives of modern medicine,pharmacology,and molecular biology,to support the use of TCM in enhancing the prevention and treatment of TIPN.
