摘  要:目的探讨中性粒细胞与淋巴细胞比值(NLR)、淋巴细胞与单核细胞比值(LMR)、血小板与淋巴细胞比值(PLR)及平均血小板体积(MPV)在骨髓增殖性肿瘤(MPN)血栓形成中的意义。方法根据确诊MPN至随访截止时间内发生血栓事件将225例MPN患者分为血栓组(36例)及无血栓组(189例),收集初诊时的基线资料并进行组间比较,采用单因素及多因素logistic回归分析探讨MPN患者发生血栓事件的危险因素。采用受试者工作特征(ROC)曲线得出相关指标对MPN血栓形成的最佳截断值并评价相关因素对MPN血栓形成的预测价值。结果血栓组年龄及MPV均高于无血栓组(P<0.05)。多因素logistic回归分析结果显示,LMR<3.67及MPV≥9.35是MPN患者发生血栓事件的独立危险因素(P<0.05)。ROC曲线分析结果示,LMR<3.67及MPV≥9.35两者联合预测MPN血栓形成的曲线下面积(AUC)均大于两者单独预测(P<0.05)。结论低LMR及高MPV与MPN血栓形成相关,LMR及MPV可作为MPN患者发生血栓事件的预测工具。Objective To investigate the significance of neutrophil to lymphocyte ratio(NLR),lymphocyte to monocyte ratio(LMR),platelet to lymphocyte ratio(PLR)and mean platelet volume(MPV)in myeloproliferative neoplasms(MPN).Methods According to the occurrence of thrombotic events from the diagnosis of MPN to the end of follow⁃up,225 MPN patients were divided into thrombus group(36 cases)and non⁃thrombus group(189 cases).Baseline data at first visit were collected and compared between groups.The risk factors of thrombotic events in MPN patients were investigated by univariate and multivariate logistic regression analysis.Receiver operating characteristic(ROC)curve was used to obtain the best cut⁃off value of relevant indicators for MPN thrombosis and to evaluate the predictive value of relevant factors for MPN thrombosis.Results The age and MPV in thrombus group were higher than those in non⁃thrombus group(P<0.05).Multivariate logistic regression analysis showed that LMR<3.67 and MPV≥9.35 were independent risk factors for thrombotic events in MPN patients(P<0.05).ROC curve analysis results showed that the area under the curve(AUC)of MPN thrombosis predicted by LMR<3.67 and MPV≥9.35 combined was greater than that predicted by them alone(P<0.05).Conclusion Low LMR and high MPV are associated with MPN thrombosis,and LMR and MPV can be used as predictive tools for thrombotic events in MPN patients.
