自拟化痰通窍熄风定痫方联合德巴金治疗儿童癫痫疗效观察

摘　　要：目的探究自拟化痰通窍熄风定痫方联合德巴金对儿童癫痫的临床疗效。方法选取2019年1月—2022年4月该院风痰闭阻型癫痫患儿150例为研究对象,按随机数字表法,将入组患儿均分为治疗组和对照组,各75例。对照组予德巴金治疗,治疗组予自拟化痰通窍熄风定痫方联合德巴金治疗,两组均持续治疗3个月。比较两组治疗后临床疗效,治疗前后中医症状积分、炎症指标[超敏C-反应蛋白(hypersensitive C-reactive protein,hs-CRP)、红细胞沉降率(erythrocyte sedimentation rate,ESR)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)]、脑电图、青少年癫痫患儿生活质量评定量表(QOLIE-AD-48)变化,及患儿癫痫发作次数、持续时间,记录治疗期间不良反应。结果治疗后,治疗组临床有效率显著高于对照组[93.33%(70/75)vs 82.67%(62/75),P<0.05];治疗后,两组中医各项积分均下降(P<0.05),且治疗组主症、次症积分均低于对照组(P<0.05);治疗后,治疗组脑电图好转总有效率86.67%(65/75)高于对照组72.00%(54/75)(P<0.05);治疗后,两组QOLIE-AD-48评分均升高(P<0.05),且治疗组高于对照组(P<0.05);治疗后,两组血清hs-CR、TNF-α、ESR水平均下降(P<0.05),且治疗组低于对照组(P<0.05);治疗后,两组癫痫发作次数及发作时持续时间均降低(P<0.05),且治疗组上述指标均低于对照组(P<0.05);治疗期间,均未见严重不良反应。结论自拟化痰通窍熄风定痫方联合德巴金可提高风痰闭阻型儿童癫痫的临床疗效,控制其症状,减轻炎症反应,有利于改善患儿脑电图指标,降低发作频率和持续时间,可提高患儿生活质量。Objective To explore the clinical efficacy of self-made Huatan Tongqiao Xifeng Dingxian Decoction(自拟化痰通窍熄风定痫方)combined with depakine on children with epilepsy.Methods A total of 150 children with epilepsy of wind-phlegm obstruction in the hospital from January 2019 to April 2022 were selected as research subjects.According to the random number table method,all enrolled children were divided into treatment group and control group,with 75 cases in each group.The control group was treated with depakine while the treatment group was given Self-made Huatan Tongqiao Xifeng Dingxian Decoction combined with depakine.Both groups were treated continuously for 3 months.The clinical efficacy after treatment,and changes in TCM symptoms scores,inflammatory indicators[hypersensitive C-reactive protein(hs-CRP),erythrocyte sedimentation rate(ESR),tumor necrosis factor-α(TNF-α)],electroencephalogram and Quality of Life in Epilepsy Inventory for Adolescents(QOLIE-AD-48)before and after treatment and the frequency and duration of epileptic seizures in children were compared between the two groups,and the adverse reactions were recorded during the treatment.Results After treatment,the clinical effective rate in the treatment group was significantly higher than that in the control group[93.33%(70/75)vs 82.67%(62/75),P<0.05].After treatment,the scores of TCM symptoms in both groups were decreased(P<0.05),and the scores of primary and secondary symptoms in the treatment group were lower than those in the control group(P<0.05).After treatment,the total effective rate of electroencephalogram improvement was 86.67%(65/75)in the treatment group and 72.00%(54/75)in the control group(P<0.05).After treatment,the QOLIE-AD-48 scores were increased in both groups(P<0.05),and the scores in the treatment group were higher than those in the control group(P<0.05).After treatment,the levels of serum hs-CR,TNF-αand ESR in both groups were reduced(P<0.05),and the levels were lower in the treatment group than those in the control
