肃金方对风邪犯肺型感染后咳嗽患者气道神经源性炎症介质的影响  

Effect of Sujin Formula(肃金方)on Airway Neurogenic Inflammatory Mediators in Patients with Cough Caused by Wind Invading Lung

摘　　要：目的探讨肃金方治疗风邪犯肺型感染性咳嗽(post-infection cough,PIC)患者临床疗效及对气道神经源性炎症介质的影响。方法将116例风邪犯肺型PIC患者随机分为治疗组、对照组各58例。对照组给予二氧丙嗪片联合肺力咳胶囊治疗,治疗组联合应用肃金方治疗。治疗1周末,比较2组中医证候积分、气道神经源性炎症介质[P物质(substance P,SP)、神经激肽A(neurokinin A,NKA)、神经激肽B(neurokinin B,NKB)、降钙素基因相关肽(calcitonin gene-related peptide,CGRP)]、生活质量、临床疗效、不良反应等指标。结果①试验过程中,对照组患者脱落剔除5例,治疗组患者脱落2例,实际完成109例。治疗组56例,对照组53例。②治疗组日间咳嗽、夜间咳嗽、咽痒、中医证候总分低于对照组(P<0.05)。③治疗组痰液SP、CGRP、NKA低于对照组(P<0.05)。④治疗组生理问题、社会问题、改良中文版莱斯特咳嗽问卷(leicester cough questionnaire,LCQ)总分高于对照组(P<0.05)。⑤治疗组总有效率94.46%(53/56)高于对照组81.13%(43/53)(χ^(2)=4.417,P<0.05)。⑥治疗组不良反应7.14%(4/56)低于对照组20.75%(11/53)(χ^(2)=4.251,P<0.05)。结论肃金方治疗能够改善风邪犯肺型PIC患者临床症状及生活质量,减少不良反应的发生,可能与抑制气道神经源炎症反应有关。Objective To study the clinical effect of Sujin Formula(肃金方)in treatment of post-infection cough(PIC)patients with wind invading lung,and its influence on airway neurogenic inflammation.Methods A total of 116 PIC patients with wind invading lung were randomly divided into treatment group(58 cases)and control group(58 cases).The control group was given dioxopromazine tablets combined with Feilike Capsule(肺力咳胶囊).The treatment group add Sujin Formula therapy.One week after treatment,the TCM syndrome scores,airway neurogenic inflammation medium[substance P(SP),neurokinin A(NKA),neurokinin B(NKB),calcitonin gene-related peptide(CGRP)],life quality,clinical efficacy and adverse reactions were compared between two groups.Result①Five patients in the control group were excluded and 2 patients in the treatment group were excluded,resulting in the completion of 109 cases.There were 56 cases in the treatment group and 53 cases in the control group.②The total scores of daytime cough,nighttime cough,pharyngeal itching and traditional Chinese medicine syndrome in the treatment group were lower than those in the control group(P<0.05).③The levels of SP,CGRP and NKA in the treatment group were lower than those of the control group(P<0.05).④Physical problems,social problems,Leicester cough questionnaire(LCQ)total score of the treatment group were higher than those the control group(P<0.05).⑤The total efficiency rate of the treatment group was 94.46%(53/56),higher than that of the control group(81.13%,43/53)(χ^(2)=4.417,P<0.05).⑥The incidence of adverse reactions of the treatment group(7.14%,4/56)was lower than that of the control group(20.75%,11/53)(χ^(2)=4.251,P<0.05).Conclusion Sujin Formula can improve the clinical symptoms and quality of life,and reduce the incidence of adverse reactions,which may be related to the inhibition of airway neurogenic inflammatory reaction.
