摘　　要：目的评价某医院各临床科室住院患者使用人血白蛋白注射液的情况,为临床合理用药提供参考。方法采用回顾性研究方法,通过该医院临床药学管理系统,随机抽取该院2022年1月至2024年1月临床各科室使用人血白蛋白的住院患者500例,对其性别、年龄、适应证、用药前后血清白蛋白指标及用药科室等方面评价用药合理性。结果在500例使用人血白蛋白的病例中,主要涉及12类疾病,其中低蛋白血症的人数最多,占比47.6%。用药合理的有261例,占总病例数的52.2%;不合理的有239例,占比47.8%,其中主要的不合理原因是用药前血清白蛋白水平大于30 g·L^(-1)、疗程过长、血清白蛋白恢复正常未停药、无用药指征及营养支持等。结论该院人血白蛋白注射液两年的使用量为7811瓶,在临床使用中存在使用不当,应严格掌握其适应证,应明确人血白蛋白使用指征并加强监管,重视循证医学证据,发挥药师的作用,提高临床合理用药水平。Objective To find out about the usage of Human Albumin injections in hospitalized patients in different clinical departments of a hospital and to evaluate the rationality of related medications.Methods From the hospital information system and using the retrospective study method,the medical records of 500 inpatients treated with Human Albumin injections between January 2022 and January 2024 were randomly selected before the rationality of usage was evaluated in terms of patients'age,departments involved,indications as well as serum albumin indexes before and after medication.Results Among the 500 cases of Human Albumin injections,12 diseases were involved.Patients with hypoproteinemia combined with other diseases accounted for 47.6%.There were 261 cases of rational drug use,accounting for 52.2%,and 239 cases of irrational use,accounting for 47.8%of the total.The major problems included too high serum albumin levels before medication,long courses of treatment,continuation of medication despite normal serum albumin,medications for wrong indications,and nutritional support.Conclusion 7800 bottles of Human Albumin injections have been used in the hospital,but there are occasions where they are not properly used.Clinicians ought to have a better knowledge of indications,care about evidence-based data and ask pharmacists for help to ensure safe use of these injections.
