预见性护理在预防老年髋部骨折患者深静脉血栓形成中的应用价值

摘　　要：目的:分析预见性护理在预防老年髋部骨折患者深静脉血栓形成(DVT)中的价值。方法:回顾性分析2022年8月—2023年8月天津市天津医院收治的70例老年髋部骨折患者资料,根据护理方法的不同分为对照组和观察组,各35例,对照组采用常规护理,观察组在对照组基础上采用预见性护理。比较两组临床指标、DVT发生情况、护理满意度、疼痛程度、DVT症状评分及心理状态评分。结果:与对照组比较,观察组骨折愈合时间、住院时间均较低,差异均有统计学意义(P<0.05)。护理后,观察组疼痛视觉模拟评分及DVT症状评分均低于对照组,差异均有统计学意义(P<0.05)。观察组DVT发生率为2.86%,低于对照组的17.14%,差异有统计学意义(P<0.05)。护理后,两组心理状态评分均较护理前降低,且观察组均低于对照组,差异均有统计学意义(P<0.05)。观察组护理满意度为97.15%,高于对照组的77.14%,差异有统计学意义(P<0.05)。结论:预见性护理能够有效预防老年髋部骨折患者发生DVT,可促进患者康复,减轻术后疼痛,同时也有效缓解了患者的负性情绪,缩短了患者住院时间。Objective:To retrospectively analyze the value of predictive nursing in preventing deep vein thrombosis(DVT)in elderly patients with hip fracture.Methods:Data of 70 elderly patients with hip fracture in Tianjin Hospital from August 2022 to August 2023 were analyzed retrospectively,and were divided into a control group and an observation group according to differen nursing methods,with 35 cases in each group.The control group received routine nursing,and the observation group received predictive nursing based on the control group.Clinical indicators,incidence of DVT,nursing satisfaction,pain degree,DVT symptam score and mental state score were compared between the two groups.Results:Compared with the control group,the fracture healing time and hospital stay in the observation group were lower,with statistical significance(P<0.05).After nursing,the pain Visual Analogue Scales and DVT symptom scores of the observation group were lower than those of the control group,the differences were statistically significant(P<0.05).The incidence of DVT in the observation group was 2.86%,which was lower than 17.14%in the control group,and the difference was statistically significant(P<0.05).After nursing,mental state scores of both groups were lower than before nusing,and the observation group were lower than the control group,with statistical significance(P<0.05).Nursing satisfaction in the observation group was 97.15%,which was higher than 77.14%in the control group,the difference was statistically significant(P<0.05).Conclusion:Predictive nursing can effectively prevent the occurrence of DVT in elderly patients with hip fracture,promote the recovery of patients,reduce postoperative pain,effectively relieve the negative emotions of patients,and shorten the length of hospital stay of patients.
