四黄散外敷联合腕踝针疗法在老年桡骨远端骨折患者护理中的临床效果观察

摘　　要：目的:探讨四黄散外敷联合腕踝针疗法在老年桡骨远端骨折(DRF)患者护理中的临床效果。方法:选择2020年1月—2024年1月泰和县中医院收治的68例老年DRF患者为研究对象,采用随机数字表法将其分为对照组和观察组,各34例。对照组予以常规护理,观察组在常规护理的基础上加用四黄散外敷联合腕踝针疗法干预。比较两组的疼痛程度、肿胀程度、恢复情况、腕关节功能、腕关节活动度、日常生活能力和生活质量。结果:干预第3、6天,观察组疼痛程度评分和肿胀程度评分均低于对照组,差异均有统计学意义(P<0.05)。观察组住院时间和骨折愈合时间均短于对照组,差异有统计学意义(P<0.05)。干预后,两组Cooney腕关节评分、腕关节活动度、日常生活能力量表评分和健康调查简表评分均高于干预前,观察组均高于对照组,差异均有统计学意义(P<0.05)。结论:四黄散外敷联合腕踝针疗法应用于老年DRF患者护理中有利于减轻患者的腕关节肿痛症状,促进腕关节功能恢复,缩短康复时间,有效提高患者的日常生活能力和生活质量。Objective:To investigate the clinical effect of Sihuang Powder external application combined with wrist and ankle acupuncture in nursing of elderly patients with distal radius fracture(DRF).Methods:A total of 68 elderly patients with DRF admitted to Taihe County Hospital of Traditional Chinese Medicine from January 2020 to January 2024 were selected as the study objects.They were divided into a control group and an observation group by random number table method,with 34 cases in each group.The control group was given routine nursing,the observation group was treated with Sihuang Powder combined with wrist and ankle acupuncture therapy on the basis of routine nursing.Pain,swelling,recovery,wrist function,wrist range of motion,ability of daily living and quality of life were compared between the two groups.Results:On the 3rd and 6th day of intervention,the pain degree and swelling degree scores of the observation group were lower than those of the control group,the differences were statistically significant(P<0.05).The hospitalization time and fracture healing time in the observation group were shorter than those in the control group,the differences were statistically significant(P<0.05).After intervention,the Cooney wrist joint score,wrist joint range of motion,daily living ability scale score and health survey summary table score of both groups were higher than before intervention,the above indicators in observation group were higher than the control group,the differences were statistically significant(P<0.05).Conclusion:The application of Sihuang Powder external application combined with wrist and ankle acupuncture therapy in nursing care of elderly patients with DRF is beneficial to alleviate the symptoms of wrist swelling and pain,promote the recovery of wrist function,shorten the recovery time,effectively improve the daily living ability and quality of life of patients.
