摘　　要：IgA肾病发病率高,呈慢性进行性发展,其具体发生发展机制尚不清楚。目前已知的影响IgA肾病不良预后因素包括蛋白尿、血尿等。高血压对影响IgA肾病预后的诸多危险因素起着不可忽视的作用。高血压通过改变肾脏血流动力学,使肾血管发生硬化、纤维化,最终损害肾功能,同时高血压状态对其他使IgA肾病进展的危险因素(胰岛素抵抗、高尿酸血症等)也有促进作用。因此,仍需要不断探索IgA肾病的发生发展机制、治疗方式,并需要在炎症免疫机制、基因变异、遗传性研究、新型检查标志物、新型治疗药物等方面进行大量研究。The incidence of IgA nephropathy is high,showing a chronic progressive development.The specific mechanism of its occurrence and development is still unclear.The currently known factors that affect the poor prognosis of IgA nephropathy include hematuria,proteinuria,etc.Hypertension plays a non-negligible role in many risk factors affecting the prognosis of IgA nephropathy.Hypertension alters renal hemodynamics,causing hardening and fibrosis of renal blood vessels,ultimately impairing renal function,while hypertension also promotes other risk factors that contribute to the progression of IgA nephropathy,such as insulin resistance and hyperuricemia.Therefore,it is still necessary to continuously explore the occurrence and development mechanisms,treatment methods of IgA nephropathy,and conduct extensive research on inflammatory and immune mechanisms,gene variations,genetic studies,novel diagnostic markers,and new therapeutic drugs.
