摘 要:目的探讨锁定加压钢板对比解剖钢板内固定治疗闭合性胫骨Pilon骨折临床疗效及远期疼痛评分是否存在差异。方法选取2016年2月-2023年8月在我院治疗的84例Pilon骨折患者分为观察组(n=42)和对照组(n=42)。观察组采用锁定加压钢板(LCP),而对照组采用解剖钢板(AP)治疗。对比两组患者出血量、手术时间、首次下地时间、住院时间,术后骨折愈合时间、术后3个月的AOFAS-AHS踝-后足关节评分、Lysholm下肢功能评分及远期VAS疼痛评分有无差异。结果观察组出血量,手术时间等优于对照组(P<0.05);观察组患者骨折愈合时间更短,AOFAS-AHS、Lysholm评分更高(P<0.05);治疗后两组患者VAS评分均开始降低,但观察组降低速率明显高于对照组,治疗后12个月,观察组VAS评分为(1.81±0.51)分,显著低于对照组的(3.82±1.10)分(P<0.05)。结论LCP内固定治疗闭合性胫骨Pilon骨折的创伤更小,术后机体功能恢复及骨折愈合更快,关节功能恢复更好,疼痛更轻。Objective To investigate the clinical efficacy and long-term pain score of compression plate vs anatomical plate internal fixation in the treatment of closed Pilon fracture of tibia.Methods 84 cases of Pilon fracture treated in our hospital from February 2016 to August 2023 were selected and divided into observation group(n=42)and control group(n=42).Observation group was treated with compression plate(LCP),while the control group was treated with anatomical plate(AP).The amount of bleeding,operation time,first landing time,postoperative fracture healing time,AOFAS-AHS ankle hindfoot score,Lysholm lower limb function score and long-term VAS pain score of the two groups were compared.Results The amount of bleeding and operation time was differnet between groups(P<0.05);the observation group had shorter fracture healing time and higher AOFAS-AHS and Lysholm scores(P<0.05);after treatment,the VAS score of the two groups began to decrease.12 months after treatment,the VAS score of the observation group was(1.81±0.51),significantly lower than that of the control group(3.82±1.10)(P<0.05).Conclusion LCP internal fixation in the treatment of closed tibial Pilon fracture has less trauma,faster recovery of body function and fracture healing,better recovery of joint function and less pain.
