固定三联药物联合肺康复治疗稳定期慢性阻塞性肺疾病的疗效观察

摘　　要：目的探讨固定三联药物联合肺康复治疗稳定期慢性阻塞性肺疾病(COPD)的治疗效果。方法选取2021年9月-2021年12月期间上海市宝山区庙行镇社区卫生服务中心医联体呼吸科病房及门诊确诊的150例稳定期COPD患者(共纳入154例,其中4例患者失访、脱落)作为研究对象,采用平行随机抽样法分为单纯治疗组与联合治疗组(三联药物+康复治疗),每组各75例。比较两组受试者肺功能指标、6分钟步行试验距离(6MWD)、呼吸困难程度、疲劳程度及生活质量差异。结果联合治疗组干预前后的肺功能指标对比,均无明显差异(均P>0.05),且其6MWD较单纯治疗组明显延长,mMRC评分、CAT评分及MFI20评分(体力疲劳、综合疲劳、动力下降、脑力疲劳、活动减少和总分)较低(均P<0.05)。结论应用固定三联药物联合肺康复治疗可更为有效地改善稳定期COPD患者的运动耐力,并显著减轻其疲劳程度,获得更高的预后质量。Objective To explore the therapeutic effect of fixed triple drugs combined with pulmonary rehabilitation in the treatment of stable chronic obstructive pulmonary disease(COPD).Methods 150 stable COPD patients diagnosed in the respiratory ward and outpatient department of the Shanghai Baoshan Miaohang Community Health Service Center from September 2021 to December 2021 were selected(a total of 154 patients were included,of which 4 patients were lost to follow-up and fell off.)as the observation object,they were randomly divided into simple treatment group and combined group(triple drug plus rehabilitation treatment)by parallel sampling method,with 75 cases in each group.The lung function index,6-minute walking distance(6MWD),dyspnea,fatigue and quality of life of the two groups were compared.Results There was no significant difference in lung function indexes between the combined group and the simple treatment group before and after the intervention(all P>0.05),and its 6MWD was significantly longer than that of the simple treatment group,and mMRC score,CAT score and MFI-20 score were lower(all P<0.05).Conclusion The application of fixed triple drugs combined with pulmonary rehabilitation can more effectively improve lung function and exercise tolerance and significantly reduce fatigue in patients with stable COPD,obtaining a higher quality of prognosis.
