药师处方干预在降低门急诊不合理处方中的效果分析

摘　　要：目的分析药师处方干预在降低门急诊不合理处方中的效果,提升临床合理用药水平。方法选取广州医科大学附属第三医院2024年第一季度门急诊处方375414张,通过药师筛查干预不合理处方,分析各月份不合理处方情况,以及不同类型不合理处方的成因。结果2024年第一季度门急诊处方不合理率为0.202%,各月分别为0.218%(1月)、0.196%(2月)、0.189%(3月)。不合理原因主要为:无间瓶或冲管/间瓶用量不足(6.46%)、重复给药(7.64%)、用量不当(5.53%)等。结论本院门急诊处方还存在一些不合理用药现象,通过门诊药师干预,临床医师处方开具的有效性、安全性和经济性得到提高,处方不合理率明显降低,临床合理用药水平提升。Objective To provide reference for clinical rational drug use and analyze the effect of pharmacist prescription intervention in reducing irrational drug use in outpatient and emergency department.Methods 375414 outpatient and emergency prescriptions in the first quarter of 2024 in our hospital were selected to intervene irrational prescriptions through pharmacist screening,and the irrational prescriptions in each month and the causes of different types of irrational prescriptions were analyzed.Results The irrationality rate of outpatient and emergency prescriptions was 0.202%in the first quarter of 2024,0.218%(January),0.196%(February)and 0.189%(March)in each month.The main reasons for irrationality were:infusion tube not swilled as required or insufficient amount of flushing(6.46%),repeated administration(7.64%),and improper dose(5.53%).Conclusion There are still some irrational drug use phenomena in outpatient and emergency prescriptions in our hospital.Through the intervention of outpatient pharmacists,the effectiveness,safety and economy of prescription in our hospital have been improved,which makes the irrational drug use rate in our hospital significantly reduced and improves the level of clinical rational drug use in our hospital.
