超声引导下和CT引导下肝内占位穿刺活检准确性对比分析

摘　　要：目的探讨超声和CT引导下肝内占位穿刺活检的价值。方法回顾性分析88例2021年3月-2024年3月本院诊断为肝占位性病变者的病历资料并将其作为研究对象。其中病理检查确诊为原发性肝癌者44例,病理检查确诊为肝脏局灶性结节增生者44例,对照组选择经CT引导下肝内占位穿刺活检,观察组选择经超声引导下肝内占位穿刺活检,分析两组对不同大小肝占位性病变检出情况,比较两组诊断价值,统计两组穿刺成功率、术后疼痛VAS评分超过3分比例及总不良反应发生率。结果观察组针对1cm以内和1-5cm肝占位性病变的检出率均高于对照组(P<0.05),观察组诊断灵敏度、特异度和准确性均高于对照组,观察组穿刺成功率高于对照组(P<0.05),术后疼痛VAS评分超过3分比例低于对照组(P<0.05),结论超声引导下肝内占位穿刺活检,具有精确度高,诊断灵敏度、特异度和准确性理想,穿刺成功率高,不良反应少等优点。Objective To explore the value of ultrasound and CT-guided intrahepatic space-occupying puncture biopsy.Methods 88 cases diagnosed as hepatic space occupying lesions from March 2021 to March 2024 in our hospital were analyzed retrospectively.Among them,44 cases were diagnosed as primary liver cancer by pathological examination,and 44 cases were diagnosed as focal nodular hyperplasia of liver by pathological examination.The control group chose CT-guided intrahepatic space-occupying puncture biopsy,while the observation group chose ultrasound-guided intrahepatic space-occupying puncture biopsy.The detection of different sizes of liver space-occupying lesions in the two groups was analyzed,the diagnostic value of the two groups was compared,and the puncture success rate,the proportion of postoperative pain VAS score exceeding 3 points and the incidence of total adverse reactions were counted.Results The detection rate of liver space occupying lesions within 1cm and 1-5cm in the observation group was higher than that in the control group(P<0.05).The diagnostic sensitivity,specificity and accuracy of the observation group were higher than those in the control group.The puncture success rate of the observation group was higher than that of the control group(P<0.05),and the postoperative pain VAS score was lower than that of the control group(P<0.05).Conclusion Ultrasound-guided liver space occupying puncture biopsy has high accuracy,diagnostic sensitivity and accuracy.
