达那唑联合rhTPO治疗对复发难治性免疫性血小板减少症患者血小板参数和免疫功能的影响

摘　　要：目的探讨达那唑联合重组人血小板生成素(rhTPO)治疗对复发难治性免疫性血小板减少症(ITP)患者血小板参数和免疫功能的影响。方法选取2022年1月至2023年5月在上饶市立医院接受治疗的76例ITP患者,采用简单随机分组将患者分为联合组(n=38)和对照组(n=38),对照组给予rhTPO治疗,联合组给予达那唑联合rhTPO治疗,两组均持续治疗2周。比较两组的血液指标[血小板计数(PLT)和血小板平均体积(MPV)],免疫细胞水平[外周血成熟T淋巴细胞(CD3^(+))、辅助性T细胞(CD4^(+))、细胞毒性T细胞(CD8^(+))],凝血功能指标[部分活化凝血活酶时间(APTT)、纤维蛋白原(FIB)、以及凝血酶原时间(PT)]及不良反应发生率。结果治疗2周后,与对照组比较,联合组MPV较低,PLT较高,差异有统计学意义(P<0.05);联合组CD4^(+)较高,CD8^(+)较低,差异有统计学意义(P<0.05)。治疗前以及治疗2周后,两组APTT、FIB、PT等凝血功能指标比较,差异无统计学意义(P>0.05),两组不良反应发生率差异无统计学意义(P>0.05)。结论达那唑联合rhTPO治疗复发难治性ITP患者可以提高其血小板计数,也可以改善免疫系统功能。Objective To investigate the effects of danazol combined with recombinant human thrombopoietin(rhTPO)on platelet parameters and immune function in patients with relapsed refractory immune thrombocytopenia(ITP).Methods A total of 76 patients with ITP who received treatment in Shangrao Municipal Hospital from January 2022 to May 2023 were selected and divided into combination group(n=38)and control group(n=38)by simple randomization.The control group was given rhTPO treatment,and the combination group was given danazol combined with rhTPO treatment,both groups were treated for 2 weeks.Blood indexes[platelet count(PLT)and mean platelet volume(MPV)],immune cell levels[peripheral blood mature T lymphocytes(CD3^(+)),helper T cells(CD4^(+)),cytotoxic T cells(CD8^(+))],coagulation function indexes[partially activated thromboplastin time(APTT),fibrinogen(FIB)and prothrombin time(PT)]and the incidence of adverse reactions were compared between the two groups.Results After 2 weeks treatment,compared with the control group,the MPV level of the combination group was lower and the PLT level was higher,which were statistically significant(P<0.05);the ITP patients in the combination group had higher levels of CD4^(+)and lower levels of CD8^(+),the differences were significant statistical(P<0.05).Before and after 2 weeks of treatment,APTT,FIB,PT and other coagulation function indexes in two groups were no statistical significance(P>0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Danazol combined with rhTPO can improve platelet count and immune system function with relapsed and refractory ITP.
