上消化道受累克罗恩病的临床分析

摘　　要：[目的]分析上消化道受累克罗恩病(CD)的临床特征、影响CD预后不良的危险因素。[方法]纳入住院诊治的CD患者143例,收集其人口学和临床资料进行分析。[结果]143例中上消化道受累CD患者63例,上消化道未受累CD患者80例。上消化道受累CD患者男女比例为2.3∶1、平均年龄为(29.4±9.3)岁。上消化道受累CD患者与上消化道未受累CD患者在L2型、L3型、红细胞沉降率、C反应蛋白、上消化道症状方面比较差异有统计学意义(P<0.05)。CD患者预后不良的因素包括男性、年龄、上消化道受累、B2型、B3型、肛周病变。[结论]上消化道受累CD患者男性多于女性,多为青壮年,病变部位以L3型为主。上消化道受累CD的预后不良,临床上应加以警惕,避免漏诊,延误治疗。[Objective]To analyze the clinical features of upper gastrointestinal(GI)tract involvement in Crohn's disease(CD)and the risk factors of poor prognosis of CD.[Methods]The CD patients from Department of Gastroenterology in Wuhan Union Hospital were included.Demographic and clinical data were collected and analyzed.[Results]Sixty-three upper GI CD patients were included.The male to female ratio in upper GI involved CD patients was 2.3:1.The mean age in upper GI involved CD patients was(29.4±9.3)years.There were significant differences in type L2,Type L3,ESR,CRP and symptoms between patients with upper GI involvement CD and those without upper GI involvement CD(P<0.05).The poor prognostic factors of CD included male,age,upper gastrointestinal involvement,type B2,Type B3,Perianal lesions.[Conclusion]Upper GI involved CD patients are more common in male than female,and most of them are young and middle-aged.The lesion site is mainly L3 type.The prognosis of upper GI involvement CD is not well,so we should pay attention to it in clinic to avoid missed diagnosis and delay treatment.
