超声下附件区包块联合血清CA125、β-hCG在诊断异位妊娠中的应用

摘　　要：目的分析超声下附件区包块联合血清糖类抗原125(CA125)、人绒毛膜促性腺激素(β-hCG)在诊断异位妊娠(EP)中的应用价值。方法回顾性选取70例EP患者,纳入EP组。选取同时期正常宫内孕者50例,纳入对照组。比较两组研究对象基线资料、附件区包块大小、环壁厚度、血流阻力指数(RI)、CA125和β-hCG水平。结果EP组RI大于对照组(P<0.05);两组附件区包块大小和环壁厚度比较,差异无统计学意义(P>0.05)。EP组CA125水平高于对照组(P<0.05),β-hCG水平低于对照组(P<0.05)。ROC曲线显示,RI、CA125和β-hCG水平预测EP的AUC分别为0.772(95%CI:0.690~0.854)、0.767(95%CI:0.683~0.851)、0.755(95%CI:0.656~0.854),三者联合预测EP的AUC为0.927(95%CI:0.878~0.976),敏感度为82.86%,特异度为92.00%,高于单独指标(Z=4.105、4.126、3.724,P=<0.001)。结论RI、CA125和β-hCG水平可作为EP的预测指标,三者联合检测可提高诊断准确性。Objective To analyze the application value of the combined use of the mass in the accessory area under ultrasound and serum carbohydrate antigen 125(CA125),human chorionic gonadotropin(β-hCG)in the diagnosis of ectopic pregnancy(EP).Methods A retrospective selection was made of 70 patients with EP which were included in the EP group.50 cases of normal intrauterine pregnancy in the same period were selected and included in the control group.The baseline data,size of the mass in the accessory area,wall thickness,blood flow resistance index(RI),CA125,andβ-hCG levels of the two groups of subjects were compared.The receiver operating characteristic curve was drawn to evaluate the predictive effect of the statistically significant difference variables on EP using the area under the curve(AUC).Results The RI in the EP group was higher than that in the control group(P<0.05);there was no statistically significant difference in the size of the mass in the accessory area and the wall thickness between the two groups(P>0.05).The CA125 level in the EP group was higher than that in the control group(P<0.05),and theβ-hCG level was lower than that in the control group(P<0.05).The ROC curve showed that the AUC of RI,CA125,andβ-hCG levels in predicting EP were 0.772(95%CI:0.690-0.854),0.767(95%CI:0.683-0.851),0.755(95%CI:0.656-0.854),respectively.The AUC of the combined prediction of the three for EP was 0.927(95%CI:0.878-0.976),with a sensitivity of 82.86%and a specificity of 92.00%,which was higher than that of the individual indicators(Z=4.105,4.126,3.724,P=<0.001,<0.001,<0.001).Conclusion RI,CA125,andβ-hCG levels can be used as predictors of EP,and the combined detection of the three can improve the diagnostic accuracy.
