加味定喘汤治疗小儿喘息性支气管炎的疗效及对免疫功能的影响

摘　　要：目的观察加味定喘汤治疗小儿喘息性支气管炎的疗效及对免疫功能的影响。方法将60例喘息性支气管炎患儿按照随机数字表法分为2组,对照组30例予抗感染、解痉平喘等治疗,治疗组30例在对照组治疗基础上联合加味定喘汤治疗。2组疗程为1周。比较2组治疗前后肺功能指标[达峰容积比(VPEF/VE)、达峰时间比(TPTEF/TE)、潮气量(VT)、呼吸频率(RR)]、中医证候评分、血清T淋巴细胞亚群(CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+))、嗜酸粒细胞(EOS)水平变化,并统计2组疗效。结果治疗组总有效率83.33%,对照组总有效率76.66%,治疗组疗效优于对照组(P<0.05)。2组治疗后中医证候各项评分均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。2组治疗后VT、RR、TPTEF/TE、VPEF/VE均较本组治疗前明显改善(P<0.05),且治疗组治疗后改善优于对照组(P<0.05)。2组治疗后CD4^(+)、CD4^(+)/CD8^(+)比值均较本组治疗前升高(P<0.05),CD8^(+)、ESO均降低(P<0.05),且治疗组治疗后CD4^(+)、CD4^(+)/CD8^(+)比值均高于对照组(P<0.05),CD8^(+)、ESO均低于对照组(P<0.05)。2组不良反应发生率比较差异无统计学意义(P>0.05)。结论加味定喘汤治疗小儿喘息性支气管炎疗效确切,可降低呼吸道炎症水平,改善肺功能,提高治疗疗效,提高患儿的免疫功能。Objective To observe the efficacy of modified Dingchuan Decoction for pediatric asthmatic bronchitis and its influence on immune function.Methods Sixty cases with pediatric asthmatic bronchitis were randomly assigned into treatment group(n=30)and control group(n=30).All patients were managed by conventional therapy(anti-infection,spasmolysis and relieving asthma),and those in treatment group were additionally treated with modified Dingchuan Decoction.Treated for 1 week,the aim was to compare pulmonary function indexes(volume to peak expiratory flow as a proportion of exhaled volume[VPEF/VE],time to peak tidal expiratory flow as a proportion of expiratory time[TPTEF/TE],tidal volume[Vt],respiratory rate[RR]),traditional Chinese medicine(TCM)syndrome score,serum T lymphocyte subsets(CD4^(+),CD8^(+),CD4^(+)/CD8^(+))and eosinophil(EOS).The curative effect was assessed.Results The overall effective rate in the treatment group was better than that in the control group(83.33%vs 76.66%,P<0.05).After treatment,TCM symptom scores in the both groups were significantly decreased(P<0.05),which decreased notably in the treatment group compared with the control group(P<0.05).VT,RR,TPTEF/TE,VPEF/VE in the both groups remarkably improved(P<0.05),the improvement in the treatment group was significantly better than the control group(P<0.05).After treatment,significantly increased CD4^(+)and CD4^(+)/CD8^(+),and decreased CD8^(+)and EOS were found in the both groups(all P<0.05),which were significantly pronounced in the treatment group than in the control group(all P<0.05).None of the incidence of adverse reactions differed significantly between groups(P>0.05).Conclusion Modified Dingchuan Decoction has a definite therapeutic effect in treating pediatric asthmatic bronchitis,which can reduce the level of respiratory inflammation,improve lung function,enhance treatment efficacy,and improve immune function.
