火针温通法治疗脾气虚型黄体功能不全的临床疗效及对月经情况、黄体功能的影响

摘　　要：目的观察火针温通法治疗脾气虚型黄体功能不全(LPD)的临床疗效及对月经情况、黄体功能的影响。方法选取脾气虚型LPD患者66例,采用随机数字表法分为2组,对照组33例予毫针治疗,观察组33例予火针治疗。对比2组临床疗效、月经情况[周期天数、经期天数、月经量失血图(PBAC)评分]、中医证候积分及黄体功能水平[基础体温高温相天数、黄体中期血清孕酮(P)水平]。结果观察组脱落2例,对照组脱落3例。观察组总有效率90.32%(28/31),对照组总有效率70.00%(21/30),观察组疗效优于对照组(P<0.05)。2组治疗后月经周期均延长,且观察组治疗后月经周期长于对照组(P<0.05)。2组治疗后经期持续天数均缩短,PBAC评分均降低,但2组治疗后经期持续天数、PBAC评分组间比较差异无统计学意义(P>0.05)。2组治疗后中医证候积分均较本组治疗前降低(P<0.05),观察组治疗后低于对照组(P<0.05)。2组治疗后高温相天数均较本组治疗前均升高(P<0.05),且观察组治疗后高于对照组(P<0.05)。2组治疗后P均较本组治疗前升高(P<0.05),但2组治疗后比较差异无统计学意义(P>0.05)。结论火针温通法可有效改善脾气虚型LPD患者月经情况及脾气虚临床证候,调节基础体温,促进血清孕激素水平恢复,健运黄体功能。Objective To explore the clinical effect of fire needle warming-dredging method on luteal phase deficiency(LPD)of spleen qi deficiency type and its influence on menstrual condition and corpus luteum function.Methods A total of 66 patients with LPD of spleen qi deficiency type were randomized 1∶1 to filiform needle treatment(control group)or fire needle treatment(observation group).The inter-group clinical effects,menstrual condition including cycle days,menstrual days,pictorial blood loss assessment chart(PBAC)score,and traditional Chinese medicine(TCM)symptom score and corpus luteum function level including high temperature phase(HTP)score of basal body temperature,serum progesterone(P)value in mid-luteal phase were compared.Results The total effective rate of observation group was significantly higher than that of control group(90.32%[28/31]vs 70.00%[21/30],P<0.05).After treatment,the menstrual cycle of the two groups were significantly longer than those before treatment(P<0.05),and which in the observation group were significantly longer than in the control group(P<0.05).Significantly shorter menstrual period and lower PBAC score were found in the two groups than those before treatment(P<0.05),but without group difference(P>0.05).After treatment,the TCM syndromes scores in the two groups were significantly lower than those before treatment(P<0.05),and the decrease in the observation group was more pronounced(P<0.05).The HTP scores and serum P values of the two groups were significantly higher than those before treatment(P<0.05),compared with the control group,the HTP score of the observation group after treatment was significantly higher(P<0.05),but serum P in the observation group was comparable to the control group(P>0.05).Conclusion Fire needle warming-dredging method can effectively improve menstrual conditions and clinical symptoms of spleen qi deficiency in patients with LPD of spleen qi deficiency type,regulate basal body temperature,promote the recovery of serum progesterone level and strengthen the
