清肺豁痰降气汤对慢性阻塞性肺疾病急性发作期患者血气指标和相关炎症因子的影响

摘　　要：目的观察清肺豁痰降气汤对慢性阻塞性肺疾病急性发作患者的治疗效果,通过对血气指标和其他实验指标的监测来为临床提供客观性依据。方法选择94例慢性阻塞性肺疾病患者为此次试验的对象,并依据治疗手段分成两组。对照组47例患者接受常规治疗,治疗组47例患者接受常规治疗联合口服清肺豁痰降气汤治疗,分别治疗14 d。相关指标:中医证候积分、治疗效果、血气指标和运动耐力、血清内的炎症因子[降钙素原(PCT)、C-反应蛋白(CRP)和白细胞介素-6(IL-6)]水平改变情况。结果总有效率方面,相较于治疗组,对照组更高(P<0.05);中医证候积分方面,相较于对照组,治疗组治疗后更低(P<0.05);治疗组治疗后血气指标动脉血氧分压(PaO_(2))、动脉血氧饱和度(SaO_(2))较对照组升高,动脉血二氧化碳分压(PaCO_(2))较对照组降低(P<0.05);且治疗组治疗后6分钟步行试验高于对照组(P<0.05);血清内的CRP、PCT和IL-6等炎症因子水平方面,治疗后治疗组相较于对照组更低(P<0.05)。不良反应出现的概率方面,两组差异无统计学意义(P>0.05)。结论清肺豁痰降气汤作为辅助治疗手段是行之有效的治疗慢性阻塞性肺疾病急性期的方法,在改善相关症状、提高运动耐力方面疗效显著,经治疗后,患者的生活质量得以提高,该治疗方法值得临床借鉴。Objective To observe the therapeutic effect of Qingfei Huotan Jiangqi Decoction on invalids with acute exacerbation of chronic obstructive pulmonary disease,and to provide objective basis for clinical practice by monitoring blood gas indexes and other experimental indexes.Methods Ninety-four chronic obstructive pulmonary disease patients were selected as the study objects,they were divided into two groups according to the treatment method.The control group(47 cases)received conventional treatment,and the treatment group(47 cases)received conventional treatment combined with oral Qingfei Huotan Jiangqi Decoction for 14 days,respectively.Related indexes:traditional Chinese medicine symptom score,therapeutic effect,blood gas indexes and exercise endurance,serum inflammatory factors[procalcitonin(PCT),C-reactive protein(CRP)and interleukin-6(IL-6)]levels.Results The total effective rate of treatment group higher than control group(P<0.05).The TCM syndrome score was lower in the treatment group than in the control group(P<0.05).After treatment,the blood gas indexes partial pressure of oxygen in arterial blood(PaO_(2))and oxygen saturation in arterial blood(SaO_(2))in treatment group were higher than control group,and partial pressure of carbon dioxide in arterial blood(PaCO_(2))was lower than control group(P<0.05).The 6 minutes walking test after treatment in treatment group was higher than control group(P<0.05).After treatment,the levels of serum inflammatory factors CRP,PCT and IL-6 in treatment group were lower than control group(P<0.05).There was no obvious difference in adverse reactions between two groups(P>0.05).Conclusion Qingfei Huotan Jiangqi Decoction,as an adjuvant treatment,is an effective method for the treatment of chronic obstructive pulmonary disease in the acute phase.It is effective in improving related symptoms and exercise endurance.After treatment,the quality of life of invalids can be significantly improved.This treatment method is worthy of clinical reference.
