活血化痰方联合颈项部腧穴深刺治疗脑卒中后吞咽障碍痰瘀互结证的效果

摘　　要：目的探讨活血化痰方联合颈项部腧穴深刺治疗脑卒中后吞咽障碍痰瘀互结证的临床效果。方法采用前瞻性随机对照分析,纳入2019年1月至2022年12月新郑天佑医院脑卒中后吞咽障碍痰瘀互结证患者86例为研究对象,按照随机数字表法分为观察组和对照组,各43例。两组均接受吞咽功能训练及常规治疗,对照组在上述基础上联合颈项部腧穴深刺治疗,观察组在对照组基础上联合活血化痰方治疗,两组均治疗4周。观察两组临床治疗效果,比较两组治疗前后吞咽能力[吞咽功能评定量表(SSA)评分]、血流变状况及不良反应发生情况。结果相较于对照组,观察组治疗4周后临床总有效率更高(P<0.05);两组血浆黏度、全血黏度高切及全血黏度低切水平均降低,且观察组低于对照组(P<0.05);两组SSA评分降低,且观察组更低(P<0.05);观察组和对照组不良反应发生率比较,差异无统计学意义(P>0.05)。结论活血化痰方联合颈项部腧穴深刺治疗脑卒中后吞咽障碍痰瘀互结证临床效果显著,可提高患者吞咽功能,改善血液流动性。Objective To explore the clinical effect of Huoxue Huatan Formula combined with deep acupuncture of neck acupoints in the treatment of post stroke swallowing disorder with intermingled phlegm-stasis blood syndrome.Methods This study used a prospective randomized controlled analysis and included 86 patients with post stroke swallowing disorder and intermingled phlegm-stasis blood syndrome at Xinzheng Tianyou Hospital from January 2019 to December 2022 as the study subjects.They were randomly divided into an observation group and a control group based on the random number table method,with 43 cases in each group.Both groups received swallowing function training and routine treatment.The control group was treated with deep acupuncture of neck acupoints on the basis of the above,while the observation group was treated with Huoxue Huatan Formula on the basis of the control group.Both groups were treated for 4 weeks.The clinical effects of the two groups were observed.The swallowing ability[swallowing function rating scale(SSA)score],blood flow degeneration and adverse reactions were compared between the two groups before and after treatment.Results Compared to the control group,the observation group had a higher overall clinical effective rate after 4 week treatments(P<0.05).The levels of plasma viscosity,whole blood viscosity high shear,and whole blood viscosity low shear in both groups were significantly reduced,and the observation group was lower than the control group(P<0.05).The SSA score of the two groups significantly decreased,and the observation group was lower than the control group(P<0.05).There was no statistical significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The combination of Huoxue Huatan Formula and deep acupuncture of neck acupoints has a significant clinical effect on the treatment of post stroke swallowing disorder with intermingled phlegm-stasis blood syndrome,which can improve patients’swallowing function,improve blood flow.
