黄芪葛根汤加减联合二甲双胍对老年2型糖尿病患者血糖水平及胰岛素抵抗的影响

摘　　要：目的分析黄芪葛根汤加减联合二甲双胍治疗老年2型糖尿病(T2DM)的效果。方法选取2020年8月—2023年8月淄博化建老年病医院收治的64例老年T2DM患者为研究对象,按随机数字表法将其分为对照组和观察组,每组32例。对照组采用二甲双胍治疗,观察组在对照组基础上采用黄芪葛根汤加减治疗。对比两组患者的临床疗效、血糖水平、血脂水平、胰岛素抵抗及不良反应发生情况。结果观察组的治疗总有效率为93.75%,高于对照组的71.88%,差异有统计学意义(P<0.05)。治疗8周后,观察组的空腹血糖、餐后2 h血糖水平、总胆固醇、甘油三酯水平分别为(6.25±0.89)mmol/L、(8.71±1.36)mmol/L,(5.38±0.95)mmol/L、(1.57±0.36)mmol/L,均低于对照组的(7.89±1.63)mmol/L、(11.58±2.01)mmol/L、(6.52±1.28)mmol/L、(2.46±0.50)mmol/L,组间差异有统计学意义(P<0.05)。治疗8周后,观察组的空腹胰岛素水平、胰岛素抵抗指数分别为(7.15±0.75)uU/mL、(4.02±0.46),均低于对照组的(8.96±1.53)uU/mL、(4.84±0.79),组间差异有统计学意义(P<0.05)。两组的不良反应发生率比较,差异无统计学意义(P>0.05)。结论黄芪葛根汤加减联合二甲双胍可降低老年T2DM患者的血糖、血脂水平,改善其胰岛素抵抗,且无严重不良反应。Objective To analyze the effect of modified Huangqi Gegen Decoction combined with metformin on elderly type 2 diabetes mellitus(T2DM).Methods A total of 64 elderly T2DM patients admitted to Zibo Huajian Geriatric Hospital from August 2020 to August 2023 were selected as the research objects and were divided into a control group and an observation group according to random number table method,with 32 cases in each group.The control group was treated with metformin,and the observation group was treated with modified Huangqi Gegen Decoction on this basis of the control group.The clinical efficacy,blood glucose levels,blood lipid levels,insulin resistance and adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 93.75%,which was higher than 71.88%of the control group,the difference was statistically significant(P<0.05).After 8 weeks of treatment,the levels of fasting blood glucose,2 h postprandial blood glucose,total cholesterol and triglyceride in the observation group were(6.25±0.89)mmol/L,(8.71±1.36)mmol/L,(5.38±0.95)mmol/L,(1.57±0.36)mmol/L,respectively,which were lower than(7.89±1.63)mmol/L,(11.58±2.01)mmol/L,(6.52±1.28)mmol/L,(2.46±0.50)mmol/L in the control group,and the differences between the groups were statistically significant(P<0.05).After 8 weeks of treatment,the fasting insulin level and homeostasis model assessment of insulin resistance in the observation group were(7.15±0.75)uU/mL and(4.02±0.46),respectively,which were lower than(8.96±1.53)uU/mL and(4.84±0.79)in the control group,and the differences between the groups were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Huangqi Gegen Decoction combined with metformin can reduce blood glucose and lipid levels and improve insulin resistance in elderly patients with T2DM without serious adverse reactions.
