超声引导下玻璃酸钠关节腔内注射联合运动疗法对肩周炎患者肩关节疼痛及功能的影响

摘　　要：目的分析超声引导下玻璃酸钠关节腔内注射联合运动疗法对肩周炎患者的影响。方法选取2021年1月—2023年10月枣庄市峄城区人民医院收治的84例肩周炎患者为研究对象,按随机数字表法将其分为对照组和观察组,各42例。对照组行运动疗法,观察组在对照组基础上采用超声引导下玻璃酸钠关节腔内注射。对比两组的临床疗效、疼痛程度、肩关节活动度、肩关节功能、生活质量。结果观察组治疗总有效率为95.24%,高于对照组的80.95%,差异有统计学意义(P<0.05)。治疗后,观察组视觉模拟评分法评分低于对照组,肩关节前屈、外展、内旋、外旋活动度均大于对照组,组间差异有统计学意义(P<0.05)。治疗后,观察组Constant-Murley肩关节功能评定量表评分与生活质量综合评定问卷内各维度评分均高于对照组,组间差异有统计学意义(P<0.05)。结论超声引导下玻璃酸钠关节腔内注射联合运动疗法可缓解肩周炎患者的肩关节疼痛,增大肩关节活动度,有利于肩关节功能的恢复,提高生活质量。Objective To analyze the effect of ultrasound-guided intra-articular injection of sodium hyaluronate combined with exercise therapy on patients with shoulder periarthritis.Methods A total of 84 patients with shoulder periarthritis admitted to Yicheng District People's Hospital in Zaozhuang City from January 2021 to October 2023 were selected as the research objects and randomly divided into a control group and an observation group,with 42 cases in each group.The control group received exercise therapy,while the observation group received under ultrasound-guided intra-articular injection of sodium hyaluronate.Compare the clinical efficacy,pain level,shoulder joint range of motion,shoulder joint function,and quality of life between two groups.Results The total effective rate of the observation group was 95.24%,which was higher than the control group's 80.95%,and the difference was statistically significant(P<0.05).After treatment,the pain Visual Analogue Scale score of the observation group was lower than that of the control group,and the range of motion of the shoulder joint in flexion,abduction,internal rotation,and external rotation was higher than that of the control group,with statistically significant differences between the groups(P<0.05).After treatment,the observation group had higher scores on the Constant Murley Shoulder Function Assessment Scale and various dimensions of the Quality of Life Comprehensive Assessment Questionnaire compared to the control group,with statistically significant differences between the groups(P<0.05).Conclusion Ultrasound-guided intra-articular injection of sodium hyaluronate combined with exercise therapy can alleviate shoulder pain in patients with shoulder periarthritis,improve shoulder range of motion,promote shoulder function recovery,and improve quality of life.
