锁定加压钢板与交锁髓内钉内固定对股骨干骨折患者围手术期指标及髋关节功能的影响  

Effects of Locking Compression Plates and Interlocking Intramedullary Nails on Perioperative Indicators and Hip Joint Function in Patients with Femoral Shaft Fractures

摘　　要：目的对比锁定加压钢板(LCP)与交锁髓内钉(IIN)内固定治疗股骨干骨折(FSF)患者的效果。方法选取2021年6月—2023年6月金乡县人民医院收治的60例FSF患者为研究对象,按随机数字表法将其分为对照组及观察组,每组30例。对照组行LCP内固定,观察组行IIN内固定。对比两组患者的围术期指标、疼痛程度、髋关节功能、生活质量及并发症发生情况。结果观察组的术中出血量为(109.68±10.42)mL,少于对照组的(153.75±14.89)mL,手术时间、住院时间、骨折愈合时间分别为(76.31±6.59)min、(10.50±1.89)d、(19.75±2.01)周,均短于对照组的(87.85±8.72)min、(14.32±2.57)d、(21.41±2.59)周,组间差异有统计学意义(P<0.05)。术后3个月,观察组的疼痛视觉模拟评分为(1.36±0.21)分,低于对照组的(2.57±0.35)分,髋关节Harris评分量表评分为(78.96±6.21)分,高于对照组的(69.84±4.69)分,组间差异有统计学意义(P<0.05);观察组的世界卫生组织生存质量测定量表简表内生理健康、心理健康、周围环境、社会关系评分分别为(76.88±4.21)分、(80.35±4.03)分、(81.26±4.13)分、(82.57±3.97)分,均高于对照组的(69.84±3.60)分、(71.26±3.28)分、(73.39±3.40)分、(72.49±3.01)分,组间差异有统计学意义(P<0.05)。观察组并发症发生率为6.67%,低于对照组的26.67%,差异有统计学意义(P<0.05)。结论IIN内固定具有更小的创伤、更短的术式时间,能够更好地减轻FSF患者疼痛,加速其髋关节功能改善,最终提升生活质量,且无严重并发症,值得临床推广使用。Objective To compare the efficacy of locking compression plates(LCP)and interlocking intramedullary nails(IIN)in the treatment of femoral shaft fracture(FSF)patients.Methods A total of 60 patients with FSF admitted to Jinxiang County People's Hospital from June 2021 to June 2023 were selected as the research objects and were divided into a control group and an observation group according to random number table method,with 30 cases in each group.The control group received LCP internal fixation,and the observation group received IIN internal fixation.The perioperative indexes,pain degree,hip function,quality of life and complications were compared between the two groups.Results The intraoperative blood loss in the observation group was(109.68±10.42)mL,which was less than(153.75±14.89)mL in the control group,while the operative time,hospital stay and fracture healing time were(76.31±6.59)min,(10.50±1.89)d and(19.75±2.01)weeks,respectively,which were shorter than(87.85±8.72)min,(14.32±2.57)d and(21.41±2.59)weeks of the control group,and the differences between the groups were statistically significant(P<0.05).3 months after surgery,the pain Visual Analogue Scale score in the observation group was(1.36±0.21)points,which was lower than(2.57±0.35)points in the control group,while the Harris hip score was(78.96±6.21)points,higher than(69.84±4.69)points in the control group,the differences between the groups were statistically significant(P<0.05);the scores of physical health,mental health,surrounding environment and social relations in the World Health Organization quality of life-BREF scale were(76.88±4.21)points,(80.35±4.03)points,(81.26±4.13)points and(82.57±3.97)points,respectively,which were higher than(69.84±3.60)points,(71.26±3.28)points,(73.39±3.40)points and(72.49±3.01)points of the control group,and the differences between the groups were statistically significant(P<0.05).The incidence of complications in the observation group was 6.67%,which was lower than 26.67%in the control group,the dif
