补肾活血消癥汤加减联合GnRH-a治疗子宫内膜异位症(寒凝血瘀证)的效果观察

摘　　要：目的 观察补肾活血消癥汤加减联合促性腺激素释放激素激动剂(GnRH-a)治疗子宫内膜异位症(EMT)(寒凝血瘀证)患者的临床效果。方法 选取90例EMT(寒凝血瘀证)患者,按照其入院顺序分为对照组和观察组,每组45例。对照组应用GnRH-a治疗,观察组在对照组基础上加补肾活血消癥汤加减治疗。比较两组治疗效果、中医证候积分、性激素水平及不良反应发生情况。结果 观察组总有效率95.56%高于对照组的82.22%(P<0.05)。治疗6个月后,两组主症、次症中医证候积分均低于本组治疗前,且观察组主症、次症中医证候积分分别为(1.42±0.27)、(1.76±0.33)分,均低于对照组的(1.96±0.42)、(2.25±0.47)分(P<0.05)。治疗6个月后,两组雌二醇(E2)、促黄体生成激素(LH)、促卵泡生成素(FSH)水平均优于本组治疗前,且观察组E2(71.39±7.85)pmol/L高于对照组的(54.16±5.71)pmol/L, LH、FSH分别为(2.31±0.76)、(11.72±1.24)IU/L,均低于对照组的(4.19±1.02)、(16.78±1.95)IU/L(P<0.05)。观察组不良反应发生率4.44%低于对照组的20.00%(P<0.05)。结论 对EMT(寒凝血瘀证)患者应用补肾活血消癥汤加减联合GnRH-a治疗效果确切,可提高临床疗效,改善患者中医证候及性激素水平,保护患者后续生育能力,还可减少不良反应,值得应用。Objective To observe the clinical effect of modified Bushen Huoxue Xiaozheng Decoction combined with gonadotropin-releasing hormone agonists(GnRH-a)in the treatment of endometriosis(EMT,cold-coagulation and blood-stasis syndrome).Methods 90 patients with EMT(cold-coagulation and blood-stasis syndrome)were selected and divided into a control group and an observation group according to their admission order,with 45 cases in each group.The control group was treated with GnRH-a,and the observation group was treated with modified Bushen Huoxue Xiaozheng Decoction on the basis of the control group.The therapeutic effect,traditional Chinese medicine syndrome score,sex hormone level and the occurrence of adverse reactions were compared between the two groups.Results The total effective rate of 95.56%in the observation group was higher than 82.22%in the control group(P<0.05).After 6 months of treatment,the traditional Chinese medicine syndrome scores for main symptom and minor symptom in both groups were lower than those before treatment;the traditional Chinese medicine syndrome scores for main symptom and minor symptom were(1.42±0.27)and(1.76±0.33)points in the observation group,which were lower than(1.96±0.42)and(2.25±0.47)points in the control group(P<0.05).After 6 months of treatment,the estradiol(E2),luteinizing hormone(LH)and follicle-stimulating hormone(FSH)in both groups were better than those before treatment;the E2 level of(71.39±7.85)pmol/L in the observation group was higher than(54.16±5.71)pmol/L in the control group;the levels of LH and FSH were(2.31±0.76)and(11.72±1.24)IU/L in the observation group,which were lower than(4.19±1.02)and(16.78±1.95)IU/L in the control group(P<0.05).The incidence of adverse reactions of 4.44%in the observation group was lower than 20.00%in the control group(P<0.05).Conclusion The application of modified Bushen Huoxue Xiaozheng Decoction combined with GnRH-a has a definite effect on patients with EMT(cold-coagulation and blood-stasis syndrome),which can improve the clin
