联合术式治疗复杂性肛瘘的临床疗效观察

摘　　要：目的 观察联合术式治疗复杂性肛瘘的临床效果。方法 58例复杂性肛瘘患者,按随机数字表法分为参照组与观察组,各29例。参照组采取切开挂线术治疗,观察组采取联合术式治疗。比较两组患者的治疗效果、创面愈合时间、视觉模拟评分法(VAS)评分及并发症发生情况、肛门失禁情况及复发情况。结果 观察组治疗后总有效率93.10%明显比参照组的72.41%高,差异存在统计学意义(P<0.05)。观察组创面愈合时间(21.27±2.36)d比参照组的(30.05±2.94)d短, VAS评分(1.24±0.15)分比参照组的(3.41±0.26)分低,差异存在统计学意义(P<0.05)。观察组并发症发生率6.90%、肛门失禁率3.45%以及复发率3.45%均低于参照组的27.59%、20.69%、20.69%,差异存在统计学意义(P<0.05)。结论 对复杂性肛瘘患者予以联合术式治疗效果甚佳,能促进患者症状的改善,加速创面愈合,缓解疼痛,减少并发症的发生,具备临床推广价值。Objective To observe the clinical effect of combined procedure in the treatment of complicated anal fistula.Methods 58 patients with complex anal fistula were divided into a reference group and an observation group according to random number table method,each with 29 cases.The reference group was treated with incision-thread-drawing procedure,and the observation group was treated with combined procedure.The therapeutic effect,wound healing time,visual analogue scale(VAS)score,occurrence of complications,anal incontinence and recurrence were compared between the two groups.Results The total effective rate of 93.10%in the observation group was significantly higher than 72.41%in the reference group,and the difference was statistically significant(P<0.05).After treatment,the observation group had shorter wound healing time of(21.27±2.36)d than(30.05±2.94)d in the reference group,and lower VAS score of(1.24±0.15)points than(3.41±0.26)points in the reference group.The difference was statistically significant(P<0.05).The observation group had complication rate of 6.90%,anal incontinence rate of 3.45%and recurrence rate of 3.45%,which were lower than 27.59%,20.69%and 20.69%in the reference group,and the differences were statistically significant(P<0.05).Conclusion In the treatment of patients with complicated anal fistula,combined procedure can promote the improvement of symptoms,accelerate wound healing,relieve pain and reduce complications,and has clinical promotion value.
