三桑化瘀补肺益肾方联合咳喘敷贴对老年肺肾气虚型慢性阻塞性肺疾病稳定期患者中医证候积分及肺功能的影响

摘　　要：目的探析三桑化瘀补肺益肾方联合咳喘敷贴对老年肺肾气虚型慢性阻塞性肺疾病(慢阻肺)稳定期患者的治疗效果。方法选取2020年1月—2023年10月同济大学附属东方医院胶州医院收治的80例老年肺肾气虚型慢阻肺稳定期患者为研究对象,根据数字奇偶法将其分为对照组和观察组,每组40例。对照组采用西医常规治疗,观察组在此基础上采用三桑化瘀补肺益肾方联合咳喘敷贴治疗。比较两组患者的临床疗效、中医证候积分、肺功能。结果观察组的治疗总有效率为95.00%,高于对照组的77.50%,差异有统计学意义(P<0.05)。治疗2个月后,观察组的咳嗽、神疲乏力、腰膝酸软评分均低于对照组,组间差异有统计学意义(P<0.05);观察组第1秒用力呼气容积(FEV1)与用力肺活量(FVC)均大于对照组,FEV1/FVC高于对照组,组间差异有统计学意义(P<0.05)。结论采用三桑化瘀补肺益肾方联合咳喘敷贴治疗老年肺肾气虚型慢阻肺稳定期患者的效果显著,可改善中医证候积分,优化肺功能,值得临床推广使用。Objective To explore the therapeutic effect of Sansang Huayu Bufei Yishen Prescription combined with cough and asthma application on elderly patients with chronic obstructive pulmonary disease(COPD)of lung-kidney Qi deficiency type in stable stage.Methods Eighty elderly patients with COPD of lung-kidney Qi deficiency type in stable stage admitted to Jiaozhou Hospital,Dongfang Hospital Affiliated to Tongji University from January 2020 to October 2023 were selected as the study objects,and were divided into a control group and an observation group according to digital parity method,with 40 cases in each group.The control group was treated with conventional western medicine,and the observation group was treated with Sansang Huayu Bufei Yishen Prescription combined with cough and asthma application on this basis.The clinical efficacy,traditional Chinese medicine syndrome scores,lung function of the two groups were compared.Results The total effective rate of the observation group was 95.00%,which was higher than 77.50%of the control group,the difference was statistically significant(P<0.05).After 2 months of treatment,the scores of cough,fatigue and waist and knee tenderness in the observation group were lower than those in the control group,and the differences between the groups were statistically significant(P<0.05);the forced expiratory volume in the first second(FEV1)and forced vital capacity(FVC)of the observation group were greater than those in the control group,and FEV1/FVC was higher that in the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion Sansang Huayu Bufei Yishen Prescription combined with cough and asthma application has a remarkable effect on the treatment of elderly patients with COPD of lung-kidney Qi deficiency type in stable stage, which can improve traditional Chinese medicine syndrome scores and optimize lung function, and is worthy of clinical popularization.
