摘 要:目的探讨早期部分负重训练联合水中步行训练对胫骨平台骨折患者肌力及关节活动度的影响。方法选取2019年6月—2023年6月商河县人民医院骨科收治的82例胫骨平台骨折患者为研究对象,按照随机数字表法将其分为对照组和观察组,每组41例。对照组进行常规训练,观察组在对照组基础上进行早期部分负重训练联合水中步行训练。比较两组患者的肌力、膝关节功能、关节活动度及生活质量。结果训练后,观察组的徒手肌力评定法、美国特种外科医院膝关节量表评分分别为(62.76±6.84)分、(68.04±12.99)分,均高于对照组的(54.86±4.11)分、(49.66±8.71)分,组间差异有统计学意义(P<0.05);观察组的膝关节屈曲角度、活动范围均大于对照组,组间差异有统计学意义(P<0.05);两组的膝关节伸直角度比较,差异无统计学意义(P>0.05);观察组的世界卫生组织生活质量评定量表简表中各维度评分均高于对照组,组间差异有统计学意义(P<0.05)。结论胫骨平台骨折患者术后进行早期部分负重训练联合水中步行训练的效果显著,可促进膝关节功能及肌力改善,增大关节活动度,提高生活质量。Objective To investigate the effect of early partial weight-bearing training combined with water walking training on muscle strength and joint range of motion in patients with tibial plateau fracture.Methods Eighty-Two patients with tibial plateau fracture admitted to the Orthopedics Department of Shanghe County People's Hospital from June 2019 to June 2023 were selected as the research objects,and were divided into a control group and an observation group according to random number table method,with 41 cases in each group.The control group received routine training,and the observation group received early partial weight-bearing training combined with water walking training based on the control group.The muscle strength,knee function,joint range of motion,and quality of life of the two groups were compared.Results After training,the Manual Muscle Test and Hospital for Special Surgery Knee scale scores of the observation group were(62.76±6.84)points and(68.04±12.99)points,respectively,which were higher than(54.86±4.11)points and(49.66±8.71)points of the control group,and the differences between the groups were statistically significant(P<0.05);the knee flexion angle and range of motion of the observation group were greater than that of the control group,and the differences between the groups were statistically significant(P<0.05);there was no significant difference in knee extension angle between the two groups(P>0.05);the World Health Organization Quality of Life-BREF scale scores of each dimension in the observation group were higher than those in the control group, and the differences between groups were statistically significant (P<0.05). Conclusion Early partial weight-bearing training combined with water walking training can improve knee function and muscle strength, increase joint motion and improve quality of life in patients with tibial plateau fracture.
