曼月乐联合戈舍瑞林治疗对子宫内膜不典型增生患者雌激素及炎症反应的影响  

Effect of Mirena Combined with Goserelin Treatment on Estrogen and Inflammatory Response in Patients with Atypical Endometrial Hyperplasia

摘　　要：目的探讨子宫内膜不典型增生(AEH)患者应用曼月乐联合戈舍瑞林的治疗效果。方法选取该院2020年7月—2022年9月收治的88例AEH患者为研究对象,应用随机数字表法将其分为对照组和研究组,各44例。对照组予以戈舍瑞林治疗,研究组在对照组基础上联合曼月乐治疗。对比两组的临床疗效、子宫内膜厚度及月经量、雌激素水平及炎性因子水平、不良反应发生情况。结果研究组治疗总有效率为95.45%,高于对照组的81.82%,差异有统计学意义(P<0.05)。治疗后,研究组月经失血图评分低于对照组,子宫内膜厚度薄于对照组,各雌激素指标(雌二醇、促黄体生成素、促卵泡生成素)水平及各炎性因子(C反应蛋白、肿瘤坏死因子-α、白介素-6)水平均低于对照组,组间差异有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论AEH患者应用戈舍瑞林联合曼月乐治疗的效果显著,有助于改善子宫内膜厚度,减少月经量,降低炎性因子水平,并可调节雌激素过度分泌现象,无明显不良反应,安全可靠,值得推广应用。Objective To explore the therapeutic effect of the combination of mirena and goserelin on patients with atypical endometrial hyperplasia(AEH).Methods A total of 88 AEH patients admitted to the hospital from July 2020 to September 2022 were selected as the research objects and randomly divided into a control group and a research group using a random number table method,with 44 patients in each group.The control group was treated with goserelin,while the research group was treated with a combination of mirena on the basis of the control group.Compare the clinical efficacy,endometrial thickness and menstrual flow,estrogen levels and inflammatory factor levels,and adverse reactions between two groups.Results The total effective rate of the research group was 95.45%,which was higher than 81.82%of the control group,and the difference was statistically significant(P<0.05).After treatment,the Pictorial Blood Loss Assessment Chart score of the research group was lower than that of the control group,and the thickness of the endometrium was smaller than that of the control group,the levels of various estrogen indicators(estradiol,luteinizing hormone,follicle stimulating hormone)and inflammatory factors(C-reactive protein,tumor necrosis factor-α,interleukin-6)of the research group were all lower than those of the control group,and the differences between groups were statistically significant(P<0.05).There was no statistically significant difference in adverse reactions between the two groups(P>0.05).Conclusion The combination of goserelin and mirena has a significant therapeutic effect on AEH patients,which helps to improve endometrial thickness,reduce menstrual flow,lower inflammatory factor levels,and regulate excessive estrogen secretion.There are no obvious adverse reactions,and it is safe and reliable.It is worth promoting and applying.
