掌根部纵行小切口与腕掌侧纵小切口腕管松解术治疗腕管综合征的有效性及安全性对比

摘　　要：目的探讨掌根部纵行小切口与腕掌侧纵小切口腕管松解术治疗腕管综合征的临床效果。方法选取2022年7月—2023年11月巨野县人民医院收治的62例腕管综合征患者为研究对象,按随机数字表法将其分为对照组和观察组,各31例。对照组行掌根部纵行小切口手术治疗,观察组行腕掌侧纵小切口手术治疗。比较两组手术情况、疼痛程度、正中神经电生理参数及生活质量。结果两组手术时间、住院时间比较,组间差异无统计学意义(P>0.05)。术前、术后1、3个月,两组疼痛视觉模拟评分比较,组间差异无统计学意义(P>0.05)。术前、术后,两组感觉神经传导速度、感觉动作电位、运动神经传导速度、复合肌肉动作电位比较,组间差异无统计学意义(P>0.05)。术前、术后,两组生活质量评分比较,组间差异无统计学意义(P>0.05)。结论掌根部纵行小切口与腕掌侧纵小切口腕管松解术在腕管综合征治疗中效果相当,均可缓解疼痛症状,改善神经电生理参数及生活质量,临床可依据情况选用。Objective To explore the clinical effect of carpal tunnel release surgery using a longitudinal small incision at the palmar root and a longitudinal small incision on the palmar side of the wrist for the treatment of carpal tunnel syndrome.Methods A total of 62 patients with carpal tunnel syndrome admitted to Juye County People's Hospital from July 2022 to November 2023 were selected as the study objects and randomly divided into a control group and an observation group,with 31 cases in each group.The control group underwent longitudinal small incision surgery at the palmar root,while the observation group underwent longitudinal small incision surgery at the wrist palmar side.The surgical conditions,pain level,median nerve electrophysiological parameters,and quality of life between two groups were compared.Results Preoperative,postoperative 1,3 months,there were no statistically significant differences between the two groups in terms of surgical time and hospital stay(P>0.05).There were no statistically significant differences in pain Visual Analogue Scale scores between the two groups(P>0.05).Before and after surgery,there were no statistically significant differences between the two groups in terms of sensory nerve conduction velocity,sensory action potential,motor nerve conduction velocity,and composite muscle action potential(P>0.05).There were no statistically significant differences in the quality of life scores between the two groups before and after surgery(P>0.05).Conclusion The longitudinal small incision at the palmar root and the longitudinal small incision at the palmar side of the wrist for carpal tunnel release are equally effective in the treatment of carpal tunnel syndrome,both of which can alleviate pain symptoms,improve neurophysiological parameters and quality of life.Clinically,they can be selected according to the situation.
