双反牵引微创复位植骨内固定治疗SchatzkerⅡ~Ⅳ型胫骨平台骨折的效果观察

摘　　要：目的探讨双反牵引微创复位植骨内固定治疗SchatzkerⅡ~Ⅳ型胫骨平台骨折的效果。方法选择靖江市中医院2021年1月—2023年8月收治的80例SchatzkerⅡ~Ⅳ型胫骨平台骨折患者为研究对象,采用电脑数字编号随机将其分为对照组和观察组,各40例。对照组采用传统切开复位内固定手术治疗,观察组采用双反牵引微创复位植骨内固定手术治疗。比较两组患者的手术情况、并发症发生情况、膝关节屈伸度、膝关节功能。结果观察组术中出血量少于对照组,手术时间、术后卧床时间、住院时间均短于对照组,组间差异有统计学意义(P<0.05)。观察组并发症发生率为7.50%,低于对照组的25.00%,差异有统计学意义(P<0.05)。术后3个月,观察组膝关节屈伸度大于对照组,差异有统计学意义(P<0.05)。术后3个月、术后6个月,观察组Lysholm膝关节功能评分均高于对照组,组间差异有统计学意义(P<0.05)。结论相比于传统切开复位内固定手术,双反牵引微创复位植骨内固定手术不仅可减轻SchatzkerⅡ~Ⅳ型胫骨平台骨折患者的手术创伤,减少术后并发症的发生,加快术后康复,还可有效地改善患者的膝关节功能和膝关节活动度。Objective Exploring the effect of double reverse traction minimally invasive reduction,bone grafting and internal fixation in the treatment of Schatzker typeⅡ~Ⅳtibial plateau fractures.Methods Selecting 80 patients with Schatzker typeⅡ~Ⅳtibial plateau fractures admitted to Jingjiang Traditional Chinese Medicile Hospital from January 2021 to August 2023 as the research subjects,randomly divide them into a control group and an observation group using computer numerical numbering,with 40 cases in each group.The control group treated with traditional open reduction and internal fixation surgery,while the observation group treated with double reverse traction minimally invasive reduction,bone grafting and internal fixation surgery.Compare the surgical situation,occurrence of complications,knee joint flexion and extension,and knee joint function between two groups of patients.Results The intraoperative bleeding volume of the observation group was less than that of the control group,and the operation time,postoperative bed rest time,and hospitalization time were all shorter than those of the control group,the differences between groups were statistically significant(P<0.05).The incidence of complications in the observation group was 7.50%,which was lower than the 25.00%in the control group,the difference was statistically significant(P<0.05).3 months after surgery,the knee joint flexion and extension in the observation group was greater than that in the control group,the difference was statistically significant(P<0.05).At 3 and 6 months after surgery,the Lysholm knee joint function scores in the observation group were higher than those in the control group,the differences between the groups were statistically significant(P<0.05).Conclusion Compared with traditional open reduction and internal fixation surgery,double reverse traction minimally invasive reduction,bone grafting and internal fixation surgery can not only reduce the surgical trauma of Schatzker typeⅡ~Ⅳtibial plateau fractures,reduce the occurrenc
