髋关节外科脱位植骨术联合复方丹参川芎方治疗早中期股骨头坏死的临床效果研究

摘　　要：目的探讨采取髋关节外科脱位植骨术与复方丹参川芎方联合方案治疗早中期股骨头坏死患者的效果。方法前瞻性收集广西贵港市中西医结合骨科医院2022年7月—2025年7月收治的60例早中期股骨头坏死住院患者为研究对象,以简单抽样法将其分为对照组和观察组,每组30例。对照组采用髋关节外科脱位植骨术治疗,观察组采用髋关节外科脱位植骨术联合复方丹参川芎方治疗。比较两组的临床治疗效果。结果观察组治疗总有效率90.00%高于对照组的70.00%,差异有统计学意义(P<0.05)。治疗后6个月,观察组血骨桥蛋白水平(49.35±10.16)ng/mL高于对照组的(43.29±10.21)ng/mL,骨保护素水平(322.04±88.14)pg/mL、骨钙素水平(28.11±2.34)ng/mL、Ⅰ型胶原C端肽水平(0.51±0.11)μg/L均低于对照组的(389.76±89.66)pg/mL、(33.44±2.21)ng/mL、(0.86±0.15)μg/L,组间差异有统计学意义(P<0.05);观察组血浆黏度(1.63±0.26)mPa.s、全血高切黏度(5.64±1.04)mPa.s、全血低切黏度(12.11±2.35)mPa.s及红细胞聚集指数(2.43±0.32)均低于对照组的(1.88±0.31)mPa.s、(8.63±1.66)mPa.s、(18.49±3.36)mPa.s、(2.88±0.45),组间差异有统计学意义(P<0.05)。治疗后6个月、8个月,观察组Harris髋关节功能评分量表、日本骨科协会评分均高于对照组,组间差异有统计学意义(P<0.05)。结论采用髋关节外科脱位植骨术联合复方丹参川芎方治疗早中期股骨头坏死患者,既可显著提高疗效,又可改善其骨代谢指标、血液流变学指标及髋关节功能,值得推荐。Objective Analyze the effect of hip joint surgical dislocation and bone grafting combined with compound Danshen Chuanxiong formula treatment on patients with early and mid stage osteonecrosis of femeral head.Methods Prospective collection of 60 hospitalized patients with early to mid stage osteonecrosis of femeral head in the Guangxi Guigang Integrated Traditional Chinese and Western Medicine Orthopedic Hospital from July 2022 to July 2025 were selected as the research objects,divided into a control group and an observation group using simple sampling method,with 30 cases in each group.The control group was treated with hip joint surgical dislocation and bone grafting,while the observation group was treated with hip joint surgical dislocation and bone grafting combined with compound Danshen Chuanxiong formula.The clinical treatment effects of the two groups were compared.Results The total effective rate of the observation group was 90.00%,which was higher than 70.00%of the control group,and the difference was statistically significant(P<0.05).After 6 months of treatment,the blood osteopontin level(49.35±10.16)ng/mL in the observation group was higher than(43.29±10.21)ng/mL in the control group,while the osteoprotegerin level(322.04±88.14)pg/mL,osteocalcin level(28.11±2.34)ng/mL,and type I collagen C-terminal peptide level(0.51±0.11)μg/L were all lower than(389.76±89.66)pg/mL,(33.44±2.21)ng/mL,and(0.86±0.15)μg/L in the control group,with statistically significant differences between the groups(P<0.05);the plasma viscosity(1.63±0.26)mPa·s,whole blood high shear viscosity(5.64±1.04)mPa·s,whole blood low shear viscosity(12.11±2.35)mPa·s,and red blood cell aggregation index(2.43±0.32)in the observation group were significantly lower than(1.88±0.31)mPa·s,(8.63±1.66)mPa·s,(18.49±3.36)mPa·s,and(2.88±0.45)in the control group,with statistically significant differences between the groups(P<0.05).After 6 and 8 months of treatment,the scores of Harris Hip Function scale and Japanese Orthopedic Associa
