复肾泄浊汤治疗慢性肾脏病脾肾两虚证的疗效观察

摘　　要：目的探讨复肾泄浊汤治疗慢性肾脏病(CKD)脾肾两虚证患者的临床疗效。方法选择2023年2月-2024年2月新疆医科大学第四临床医学院的120例CKD脾肾两虚证患者,按随机数字表法分为中药组(60例)和对照组(60例)。对照组接受常规西医治疗,中药组则在此基础上加用复肾泄浊汤,治疗周期为12周。比较两组中医证候评分、临床疗效、肾功能指标[血肌酐(Scr)、尿素氮(BUN)、尿酸(UA)、估算肾小球滤过率(eGFR)]和不良反应发生情况。结果中药组59例,对照组57例,共116例。中药组总有效率为91.50%(53/59),对照组为73.68%(42/57)。治疗12周后,中药组腰膝酸软、倦怠乏力、纳呆腹胀、浮肿难消较对照组显著改善(P<0.05)。中药组Scr、BUN低于对照组,肾小球滤过率(eGFR)高于对照组(P<0.05)。两组尿酸(UA)差异无统计学意义(P>0.05)。治疗期间两组均无严重不良反应。结论CKD脾肾两虚证患者常规西医治疗的基础上加用复肾泄浊汤治疗可有效改善肾功能,可延缓肾功能恶化的趋势,临床疗效显著。Objective To investigate the clinical effect of Fushen Xiezhuo decoction on chronic kidney disease(CKD)patients with spleen and kidney deficiency syndrome.Methods A total of 120 patients with spleen and kidney deficiency syndrome of CKD in the hospital from February 2023 to February 2024 were selected and divided into traditional Chinese medicine(TCM)group(60 cases)and control group(60 cases)according to the random number table method.The control group received conventional Western medicinetreatment,and TCM group was treated with Fushen Xiezhuo decoction.The treatment period was 12 weeks.TCM syndrome score,clinical efficacy,renal function indexes[serum creatinine(Scr),urea nitrogen(BUN),uric acid(UA),glomerular filtration rate(eGFR)]and adverse reactions were compared between the 2 groups.Results There were 59 patients in TCM group and 57 patients in control group.A total of 116 patients completed the study.The total effective rate was 91.50%(53/59)in TCM group and 73.68%(42/57)in control group.After 12 weeks of the treatment,the waist and knee sore-ness,fatigue,abdominal distension and edema in TCM group were significantly improved compared with control group(P<0.05).The serum creatinine(Scr)and blood urea nitrogen(BUN)in TCM group were lower than those in control group,and the glomerular filtration rate(eGFR)was higher than that in con-trol group(P<0.05).There was no significant difference in uric acid(UA)between the 2 groups(P>0.05).There were no serious adverse reactions during the treatment.Conclusion On the basis of conven-tional Western medicine treatment for the patients with CKD of spleen and kidney deficiency,Fushen Xiezhuo decoction can effectively improve renal function,delay the deterioration of renal function,and the clinical effect is more significant.
