黄芪益肾颗粒对阳虚证Ⅱ型心肾综合征的临床疗效

摘　　要：目的 研究黄芪益肾颗粒对阳虚证Ⅱ型心肾综合征(CRS)的临床疗效。方法 采用前瞻性随机对照研究,按照随机数表法将广东省第二中医院白云院区心血管科2021年9月至2023年8月收治的110例阳虚证Ⅱ型CRS患者分为两组,每组各55例。对照组接受常规西医治疗,治疗组在对照组基础上加用芪益肾颗粒1包tid治疗,4周为1个疗程,治疗2个疗程。比较两组中医症候积分、心功能指标、肾功能指标、炎症指标及不良反应发生情况。结果 治疗2个疗程后,治疗组中医症候积分低于对照组,差异有统计学意义(P <0.05);治疗组左心室射血分数、左室短径缩短率、舒张早期峰值/舒张晚期峰值高于对照组,差异有统计学意义(P <0.05);治疗组胱抑素C、血肌酐、尿素氮水平低于对照组,差异有统计学意义(P <0.05);治疗组白细胞计数、中性粒细胞计数、超敏C反应蛋白水平低于对照组,差异有统计学意义(P <0.05);治疗期间两组均无明显不良反应。结论黄芪益肾颗粒可改善阳虚证Ⅱ型CRS患者中医症候及心肾功能,减轻炎症反应,且安全性高,值得临床推广。Objective To study the clinical efficacy of Huangqi Yishen granules in treating typeⅡcardiorenal syndrome(CRS)with Yang deficiency syndrome.Methods A prospective randomized controlled study was conducted in this study.A random number table method was used to divide 110 patients with typeⅡCRS of Yang deficiency syndrome in the Cardiovascular Department,Guangdong Second Traditional Chinese Medicine Hospital(Baiyun District)from September 2021 to August 2023 into two groups,with 55 cases in each group.The control group received routine western medicine treatment,while the treatment group was treated with Qiyishen granules 1 pack of tid on the basis of the control group,with a treatment course of 4 weeks.The traditional Chinese medicine syndrome score,heart function index,kidney function index,inflammation index,and incidence of adverse reactions were compared between the two groups.Results After 2 courses of treatment,the scores of traditional Chinese medicine symptoms in the treatment group were lower than those in the control group,and the difference was statistically significant(P<0.05).The left ventricular ejection fraction,ventricular shortening rate,and early diastolic peak velocity/late diastolic tissue velocity in the treatment group were higher than those in the control group,and the differences were statistically significant(P<0.05).The levels of cystain C,serum creatinine and blood urea nitrogen in the treatment group were lower than those in the control group,and the differences were statistically significant(P<0.05).The levels of white blood cell count,neutrophil count and hypersensitivity C-reactive protein in the treatment group were lower than those in the control group,and the differences were statistically significant(P<0.05).There were no obvious adverse reactions in both groups during treatment.Conclusion Huangqi Yishen granules can improve traditional Chinese medicine syndrome and heart and kidney function in typeⅡCRS patients with Yang deficiency syndrome,reduce inflammatory reactions,and
