富血小板血浆联合低强度脉冲超声治疗膝关节骨性关节炎患者的效果分析

摘　　要：目的:探讨关节内注射富血小板血浆(PRP)联合低强度脉冲超声(LIPUS)对膝关节骨性关节炎(KOA)患者的临床疗效。方法:选择2019年8月—2021年8月徐州医科大学附属淮安医院收治的100例轻度至中度KOA患者为研究对象,随机将其分为对照组与研究组,各50例。对照组采用PRP关节内注射治疗,研究组在此基础上联合LIPUS治疗。对比两组疼痛程度、膝关节功能、疾病严重程度、临床疗效、不良反应发生情况及膝关节软骨恢复情况。结果:治疗后6个月,两组疼痛视觉模拟评分、Lequesne指数评分、WOMAC量表中疼痛和日常生活能力评分均低于治疗前,且研究组均低于对照组,差异均有统计学意义(P<0.05)。治疗后6个月,两组WOMAC量表中僵硬评分低于治疗前,差异有统计学意义(P<0.05)。但两组组间比较差异无统计学意义(P>0.05)。治疗后6个月,研究组的治疗总有效率为96.00%,不良反应发生率为2.00%,相较于对照组的86.00%和0.00%,差异均无统计学意义(P>0.05)。结论:关节内注射PRP联合LIPUS对KOA的临床疗效确切,可有效缓解KOA患者疼痛程度及疾病严重程度,促进其关节功能及软骨厚度恢复,且安全性较高。Objective:To investigate the clinical efficacy of intra-articular injection of platelet-rich plasma(PRP)combined with low intensity pulsed ultrasound(LIPUS)in patients with knee osteoarthritis(KOA).Methods:A total of 100 patients with mild to moderate KOA admitted to Huai’an Hospital Affiliated to Xuzhou Medical University from August 2019 to August 2021 were selected as the study objects,and randomly divided into a control group and a study group,with 50 cases in each group.The control group was treated with PRP intra-articular injection,the study group was treated with LIPUS on this basis.The pain degree,knee function,disease severity,clinical efficacy,occurrence of adverse reactions and knee cartilage recovery of the two groups were evaluated.Results:After 6 months of treatment,pain visual analogue score,Lequesne index score and WOMAC scale score of pain and daily living ability were lower in the two groups than before treatment,and the study group were lower than the control group,the differences were statistically significant(P<0.05).After 6 months of treatment,the stiffness score of the WOMAC scale in the two groups was lower than that before treatment,the difference was statistically significant,but there was no statistical significance between the two groups(P>0.05).After 6 months of treatment,the total effective rate of the study group was 96.00%and the incidence of adverse reactions was 2.00%,which had no statistical significance compared with 86.00%and 0.00%of the control group(P>0.05).Conclusion:Intra-articular injection of PRP combined with LIPUS has a definite clinical effect on KOA,which can effectively relieve the pain degree and disease severity of KOA patients,promote the recovery of joint function and cartilage thickness,and is safe.
