超声引导下内热针治疗肩背肌筋膜炎患者的效果分析

摘　　要：目的:探讨超声引导下内热针治疗肩背肌筋膜炎患者的效果及应用价值。方法:选取2023年3—11月昌邑市人民医院收治的92例肩背肌筋膜炎患者为研究对象,按随机数字表法将其分为对照组和研究组,各46例。对照组采用体外冲击波治疗,研究组在对照组基础上采用超声引导下竖脊肌间隙内热针治疗。比较两组症状积分、日常活动能力、炎症水平及临床疗效。结果:研究组治疗总有效率为97.83%,高于对照组的84.78%,差异有统计学意义(P<0.05)。治疗后,两组压痛、肿胀、功能障碍症状积分、白细胞介素-6、C反应蛋白水平均较治疗前降低,且研究组均低于对照组,差异均有统计学意义(P<0.05)。治疗1、2、4周后,两组改良Barthel指数(MBI)评分均较治疗前升高,且治疗2、4周后研究组MBI评分均高于对照组,差异均有统计学意义(P<0.05)。结论:肩背肌筋膜炎患者采用超声引导下内热针疗法,能够提升治疗的总有效率,改善压痛、肿胀、功能障碍等症状,提高日常生活能力,缓解体内炎症反应。Objective:To investigate the effect and application value of ultrasound-guided internal heat acupuncture in the treatment of shoulder and back myofascitis.Methods:A total of 92 patients with shoulder and back myofascitis treated in Changyi People’s Hospital from March to November 2023 were selected as the study objects.They were divided into a control group and a study group according to random number table method,with 46 cases in each group.The control group was treated with extracorporeal shock wave therapy,the study group was treated with heat acupuncture in the erector spinal space guided by ultrasound on the basis of the control group.Symptom score,daily activity ability,inflammation level and clinical efficacy were compared between the two groups.Results:The total effective rate of the study group was 97.83%,which was higher than 84.78%of the control group,the difference was statistically significant(P<0.05).After treatment,symptom scores of tenderness,swelling,dysfunction,levels of interleukin-6 and C-reactive protein in both groups were lower than before treatment,the study group were lower than the control group,with statistical significance(P<0.05).After 1,2 and 4 weeks of treatment,the Modified Barthel Index(MBI)score of both groups was higher than before treatment,the MBI score of the study group were higher than those of the control group after 2 and 4 weeks of treatment,with statistical significance(P<0.05).Conclusion:The use of ultrasound-guided internal heat acupuncture therapy in patients with shoulder and back myofascitis can improve the total effective rate of treatment,improve the symptoms of tenderness,swelling,dysfunction,improve the ability of daily life,and alleviate the inflammation in the body.
